No seizures or unconsciousness had been noted. The time between episodes of weakness and collapse was variable, ranging from weekly to once every 3 to 4 weeks. Blue would no longer go on walks further than a mile.<\/p>\n<p>No change in water consumption, appetite, urination, or defecation had been noted. Blue&#8217;s food consisted of a senior dry kibble fed twice daily along with several treats per day; she had not had any incidences of vomiting or diarrhea. Blue was not currently receiving any medications.<\/p>\n<h2>PHYSICAL EXAMINATION<\/h2>\n<p>Blue was obese with a body condition score of 8 out of 9. Vitals were within normal limits. There were no murmurs or arrhythmias, masses or organomegaly, or muscle atrophy. Peripheral lymph nodes were &lt; 1 cm.<\/p>\n<h2><span class=\"redheader\">LABORATORY ANALYSIS<\/span><\/h2>\n<p>The initial laboratory evaluation consisted of a CBC, serum biochemical profile, and urinalysis. The CBC and urinalysis were unremarkable. Serum biochemical profile results included (<strong>Table 1<\/strong>):<\/p>\n<ul>\n<li>Mild increase in hepatocellular enzymes\u2014alanine aminotransferase (ALT) and aspartate aminotransferase (AST)<\/li>\n<li>Normal alkaline phosphatase (ALP)<\/li>\n<li>Marked hypoglycemia.<\/li>\n<\/ul>\n<p><img fetchpriority=\"high\" decoding=\"async\" src=\"http:\/\/216.119.71.215\/mags\/1205\/c03_table1.gif\" alt=\"\" width=\"400\" height=\"214\" \/><\/p>\n<h2><span class=\"redheader\">DIFFERENTIAL DIAGNOSIS<\/span><\/h2>\n<p>Based on history, signalment, and physical examination (including body condition), the primary differential diagnoses for hypoglycemia and episodic weakness (<strong>Table 2<\/strong>) in this dog included:<\/p>\n<ul>\n<li>Insulin-secreting beta-cell tumor (insulinoma)<\/li>\n<li>Extrapancreatic neoplasia (eg, hepatoma, leiomyoma, leiomyosarcoma, lymphosarcoma, etc)<\/li>\n<li>Atypical hypoadrenocorticism (Addison&#8217;s disease).<\/li>\n<\/ul>\n<p><img decoding=\"async\" src=\"http:\/\/216.119.71.215\/mags\/1205\/c03_table2.gif\" alt=\"\" width=\"580\" height=\"500\" \/><\/p>\n<p><a href=\"https:\/\/todaysveterinarypractice.com\/wp-content\/uploads\/sites\/4\/2016\/03\/050612-p2p-what-you-need-copy.jpg\"><img loading=\"lazy\" decoding=\"async\" class=\"alignnone wp-image-9825\" src=\"https:\/\/todaysveterinarypractice.com\/wp-content\/uploads\/sites\/4\/2016\/03\/050612-p2p-what-you-need-copy-430x1024.jpg\" alt=\"EpubXpress - Text PDF\" width=\"350\" height=\"834\" srcset=\"https:\/\/navc.sitepreview.app\/todaysveterinarypractice.com\/wp-content\/uploads\/sites\/4\/2016\/03\/050612-p2p-what-you-need-copy-430x1024.jpg 430w, https:\/\/navc.sitepreview.app\/todaysveterinarypractice.com\/wp-content\/uploads\/sites\/4\/2016\/03\/050612-p2p-what-you-need-copy-126x300.jpg 126w, https:\/\/navc.sitepreview.app\/todaysveterinarypractice.com\/wp-content\/uploads\/sites\/4\/2016\/03\/050612-p2p-what-you-need-copy-768x1830.jpg 768w, https:\/\/navc.sitepreview.app\/todaysveterinarypractice.com\/wp-content\/uploads\/sites\/4\/2016\/03\/050612-p2p-what-you-need-copy-645x1536.jpg 645w, https:\/\/navc.sitepreview.app\/todaysveterinarypractice.com\/wp-content\/uploads\/sites\/4\/2016\/03\/050612-p2p-what-you-need-copy-860x2048.jpg 860w, https:\/\/navc.sitepreview.app\/todaysveterinarypractice.com\/wp-content\/uploads\/sites\/4\/2016\/03\/050612-p2p-what-you-need-copy.jpg 1001w\" sizes=\"(max-width: 350px) 100vw, 350px\" \/><\/a><\/p>\n<h2 class=\"wp-caption-dd\">ADDITIONAL DIAGNOSTICS<\/h2>\n<h3><span class=\"bold\">Serum Insulin Concentration<\/span><\/h3>\n<p>Serum insulin concentration, from the blood sample demonstrating hypoglycemia (35 mg\/dL), was significantly increased (&gt; 310 mcU\/mL; reference interval, 5-20 mcU\/mL) and consistent with a diagnosis of an insulin-secreting beta-cell tumor.<\/p>\n<h3><span class=\"bold\">Diagnostic Imaging<\/span><\/h3>\n<p>Thoracic radiographs were taken to determine if metastatic lesions were present; there were no significant abnormalities.<\/p>\n<p>Abdominal ultrasound revealed a markedly enlarged (1.5 cm) mesenteric lymph node in the right cranial abdomen near the caudal vena cava, which was consistent with a metastatic lesion, and a 0.5-cm hypoechoic nodule in the liver, which could represent metastatic disease or benign change. No definitive pancreatic nodules were identified (<strong>Figure 1<\/strong>).<\/p>\n<p>Computed tomography (CT) was not performed.<\/p>\n<p><img loading=\"lazy\" decoding=\"async\" src=\"http:\/\/216.119.71.215\/mags\/1205\/c03_fig01.jpg\" alt=\"\" width=\"344\" height=\"392\" \/><\/p>\n<p><a href=\"https:\/\/todaysveterinarypractice.com\/wp-content\/uploads\/sites\/4\/2016\/03\/050612-p2p-insulinoma.jpg\"><img loading=\"lazy\" decoding=\"async\" class=\"alignnone wp-image-9826\" src=\"https:\/\/todaysveterinarypractice.com\/wp-content\/uploads\/sites\/4\/2016\/03\/050612-p2p-insulinoma-731x1024.jpg\" alt=\"EpubXpress - Text PDF\" width=\"475\" height=\"666\" srcset=\"https:\/\/navc.sitepreview.app\/todaysveterinarypractice.com\/wp-content\/uploads\/sites\/4\/2016\/03\/050612-p2p-insulinoma-731x1024.jpg 731w, https:\/\/navc.sitepreview.app\/todaysveterinarypractice.com\/wp-content\/uploads\/sites\/4\/2016\/03\/050612-p2p-insulinoma-214x300.jpg 214w, https:\/\/navc.sitepreview.app\/todaysveterinarypractice.com\/wp-content\/uploads\/sites\/4\/2016\/03\/050612-p2p-insulinoma-768x1076.jpg 768w, https:\/\/navc.sitepreview.app\/todaysveterinarypractice.com\/wp-content\/uploads\/sites\/4\/2016\/03\/050612-p2p-insulinoma-1096x1536.jpg 1096w, https:\/\/navc.sitepreview.app\/todaysveterinarypractice.com\/wp-content\/uploads\/sites\/4\/2016\/03\/050612-p2p-insulinoma-1462x2048.jpg 1462w, https:\/\/navc.sitepreview.app\/todaysveterinarypractice.com\/wp-content\/uploads\/sites\/4\/2016\/03\/050612-p2p-insulinoma-scaled.jpg 1827w\" sizes=\"(max-width: 475px) 100vw, 475px\" \/><\/a><\/p>\n<h2 class=\"wp-caption-dd\">SURGICAL TREATMENT<\/h2>\n<p>Surgical exploration of the abdomen found nodules in the right and left limbs of the pancreas. The nodule in the right limb was resected and the majority of the left limb of the pancreas was removed (<strong>Figure 2<\/strong>). Several mesenteric lymph nodes were enlarged and also removed (<strong>Figure 3<\/strong>).<\/p>\n<p><img loading=\"lazy\" decoding=\"async\" src=\"http:\/\/216.119.71.215\/mags\/1205\/c03_fig02-03.jpg\" alt=\"\" width=\"344\" height=\"656\" \/><\/p>\n<h3><span class=\"bold\">Histopathology<\/span><\/h3>\n<p>Histopathology confirmed:<\/p>\n<ul>\n<li>Infiltrative beta cell carcinoma in the left limb of the pancreas (<strong>Figures 4 and 5<\/strong>)<\/li>\n<li>Metastatic beta cell carcinoma in the mesenteric lymph nodes.<\/li>\n<\/ul>\n<p><img loading=\"lazy\" decoding=\"async\" src=\"http:\/\/216.119.71.215\/mags\/1205\/c03_fig04-05.jpg\" alt=\"\" width=\"344\" height=\"683\" \/><\/p>\n<p>Neoplastic cells in both the pancreas and mesenteric lymph node were strongly immunoreactive to antibodies against insulin, confirming insulin-secreting beta-cell carcinoma (<strong>Figure 6<\/strong>). No evidence of neoplasia was noted in the right limb of the pancreas or the hepatic biopsy.<\/p>\n<p><img loading=\"lazy\" decoding=\"async\" src=\"http:\/\/216.119.71.215\/mags\/1205\/c03_fig06.jpg\" alt=\"\" width=\"344\" height=\"365\" \/><\/p>\n<h3><span class=\"bold\">Post Operative Recovery<\/span><\/h3>\n<p>Blue recovered from anesthesia without complications. Blood glucose measurements stabilized at 70 to 100 mg\/dL while in the hospital post operatively. No specific medical therapy was started postoperatively except for routine postoperative care, including IV fluids, pain medication, antiemetics, and small frequent meals of a low-fat diet.<\/p>\n<h2>FOLLOW-UP<\/h2>\n<p>Blue was discharged 4 days after surgery. She was initially fed a low-fat food (to reduce risk of pancreatitis post operatively) three times a day until the 1-month recheck. A follow-up was recommended in 10 to 14 days for suture removal and to recheck her blood glucose.<\/p>\n<h3><span class=\"bold\">Two-Week Recheck<\/span><\/h3>\n<p>At the 14-day recheck, Blue&#8217;s incision had healed nicely and blood glucose measurements were 66 and 71 mg\/dL.<\/p>\n<h3><span class=\"bold\">One-Month Recheck<\/span><\/h3>\n<p>At the 1-month recheck, Blue&#8217;s blood glucose, which was measured at home, ranged from 50 to 80 mg\/dL, with occasional signs compatible with hypoglycemia. The owners noted that Blue had improved significantly since surgery, but recurrence of clinical signs with concurrent low blood glucose measurements confirmed metastatic disease containing functional islet cells that were probably producing insulin.<\/p>\n<p>Blue&#8217;s food was switched to a diet with a moderate fat content, a mix of insoluble and soluble fiber, and limited simple sugars; she was fed 4 times a day.<\/p>\n<h3><span class=\"bold\">Four-Month Recheck<\/span><\/h3>\n<p>At the 4-month recheck, most of Blue&#8217;s blood glucose measurements were approximately 50 mg\/dL; however, her measurements were 70 to 80 mg\/dL in the evenings. Blue was prescribed prednisone, 2.5 mg (0.16 mg\/kg) PO Q 12 H, and fed 4 meals a day.<\/p>\n<h2>OUTCOME<\/h2>\n<p>Blue was last evaluated 21 months following surgery. She had measured blood glucose concentrations of 50 to 80 mg\/dL and was being maintained on prednisone (2.5 mg PO Q 12 H), with minimal hypoglycemic complications.<\/p>\n<p>ALP = alkaline phosphatase; ALT = alanine aminotransferase; AST = aspartate aminotransferase;\u00a0CT = computed tomography<\/p>\n<h3 class=\"references\"><strong>References<\/strong><\/h3>\n<ol>\n<li class=\"references\">Polton GA, White RN, Brearley MJ, et al.
