The prevalence of CKD has been estimated to be 0.5% to 1% in dogs and 1% to 3% in cats,<sup>1,2<\/sup> but it increases with age, especially in cats. An estimated 30% to 50% of cats 15 years of age or older have CKD.<sup>3-5<\/sup><\/p>\n<p>Nephron damage associated with CKD is irreversible and often progressive. Nephrocalcinosis, systemic hypertension, intraglomerular hypertension, and proteinuria have been associated with progression of CKD.<sup>6-9<\/sup> All of these mechanisms of renal disease progression are potentially treatable.<\/p>\n<p>Although not proven by prospective, controlled clinical trials, it is logical to assume that early diagnosis\u2014followed by renoprotective treatment of CKD\u2014can slow its progression in dogs and cats. Firm evidence shows that dietary management can slow the progression of CKD, and increasing evidence suggests that antiproteinuric treatments also have this effect.<sup>10-14<\/sup><\/p>\n<p><strong>Learn More: <\/strong>The International Renal Interest Society provides guidelines for both staging and treatment of canine and feline CKD, which are available at <strong>iris-kidney.com\/guidelines\/staging.aspx<\/strong> and <strong>iris-kidney.com\/guidelines\/recommendations.aspx<\/strong>, respectively.<\/p>\n<div class=\"orange-box\">\n<h2>Serum Creatinine Concentration<\/h2>\n<\/div>\n<p>Early, nonazotemic CKD\u2014International Renal Interest Society (IRIS) stage 1\u2014in dogs and cats may be associated with:<\/p>\n<ul>\n<li>Abnormal renal palpation or renal imaging findings<\/li>\n<li>Persistent renal proteinuria<\/li>\n<li>Urine concentrating deficits due to renal disease<\/li>\n<li>Persistent increases in serum symmetric dimethylarginine (SDMA) concentrations due to renal disease<\/li>\n<li>Decreases in glomerular filtration rate (GFR) due to renal disease.<\/li>\n<\/ul>\n<p>Plasma clearance techniques (eg, iohexol plasma clearance) that do not require collection of urine have led to simplified measurement of GFR in recent years. However, due to expense, time, and labor, GFR is rarely measured in the practice setting.<\/p>\n<h3>Relationship with Glomerular Filtration Rate<\/h3>\n<p>Serum creatinine (sCr) concentrations are commonly used as a surrogate for GFR because they have an inverse relationship. In early CKD, relatively large changes in GFR are associated with small changes in sCr concentration, while the opposite is true in later stages of CKD. Unfortunately, this relationship is not linear (<strong>Figure 1<\/strong>), which limits the sensitivity of sCr for detection of early renal disease.<\/p>\n<div id=\"attachment_9176\" style=\"width: 310px\" class=\"wp-caption aligncenter\"><a href=\"https:\/\/todaysveterinarypractice.com\/wp-content\/uploads\/sites\/4\/2016\/02\/T1603F05Fig01.jpg\"><img fetchpriority=\"high\" decoding=\"async\" aria-describedby=\"caption-attachment-9176\" class=\"size-medium wp-image-9176\" src=\"https:\/\/todaysveterinarypractice.com\/wp-content\/uploads\/sites\/4\/2016\/02\/T1603F05Fig01-300x231.jpg\" alt=\"FIGURE 1. It is well known that highly muscled breeds, such as greyhounds, have higher sCr concentrations than do breeds with less muscle. Renal function may be overestimated in aging animals with reduced muscle mass and, therefore, reduced sCr concentrations.<\/p>\n<h3>Method of Analysis<\/h3>\n<p>Interpretation of sCr concentrations can also be influenced by the method of analysis (Jaffe reaction versus enzymatic reaction; benchtop versus reference laboratory). Comparing sCr concentrations from the same patient when more than one laboratory or method is involved does not always provide an \u201capples to apples\u201d comparison.<\/p>\n<p>One of the most disconcerting aspects of sCr interpretation is the relatively large variation in reference intervals between laboratories, which can lead to false-positive and false-negative diagnoses of azotemia.<sup>15,16<\/sup> Reference intervals need to be individualized to each laboratory, but many veterinary nephrologists have suggested that a threshold is crossed over at IRIS stage 2 CKD:<\/p>\n<ul>\n<li>This stage begins with sCr concentrations within the upper range of most reference intervals: 1.4 and 1.6 mg\/dL in dogs and cats, respectively<sup>17<\/sup><\/li>\n<li>A sCr concentration of 1.4 mg\/dL or greater in a dog or 1.6 mg\/dL or greater in a cat does not necessarily indicate azotemia, but it does suggest that the patient should be monitored more closely.<\/li>\n<\/ul>\n<p>sCr concentrations must always be interpreted in light of the patient\u2019s muscle mass, urine specific gravity (USG), and physical examination findings in order to rule out volume-responsive and postrenal causes of azotemia. For example, a high normal sCr concentration in an older cat with loss of muscle mass may indicate decreased renal excretory function. Conversely, a high sCr concentration in a dehydrated dog with a USG of 1.040 would suggest prerenal azotemia.<\/p>\n<h3>Longitudinal Measurement<\/h3>\n<p>Despite all these potential pitfalls, longitudinal assessment of sCr concentrations (analyzed by consistent methods) is an excellent tool to assess renal function and diagnose early CKD. For example, a sCr concentration that increases from 0.7 to 1.4 mg\/dL over several years in a patient without evidence of dehydration or an increase in muscle mass could indicate a minimum 50% reduction in GFR. The reduction in renal function may be greater than 50% due to compensatory hypertrophy of remaining nephrons. Increases in sCr of at least 0.3 mg\/dL are more than would be expected with normal assay variation when the same method is used.<\/p>\n<h2>SDMA Concentration<\/h2>\n<p>SDMA is derived from intranuclear methylation of L-arginine by protein-arginine methyltransferases; it is released into the blood after proteolysis. SDMA is eliminated primarily by glomerular filtration and is not affected by tubular reabsorption or secretion. Therefore, it can be used as an intrinsic GFR marker.<\/p>\n<p>Several studies in humans have documented the utility of serum SDMA concentrations as a biomarker of renal function; a meta-analysis of 18 studies involving more than 2100 humans documented a high correlation of SDMA to both GFR and sCr.<sup>18<\/sup><\/p>\n<h3>Role as a Biomarker<\/h3>\n<p>Recently, SDMA has received attention in veterinary medicine as a kidney excretory function biomarker.<\/p>\n<p><strong>Cats.<\/strong> One of the first studies involving aged, client-owned cats reported an inverse linear relationship between serum SDMA concentration and GFR (iohexol plasma clearance) (<em>R<sup>2<\/sup><\/em> = 0.82; <em>P<\/em> &lt; 0.001).<sup>19<\/sup><\/p>\n<p>In addition to correlating well with GFR, serum SDMA may be a more sensitive biomarker for detection of early CKD compared with sCr. In longitudinal studies in cats that developed CKD, SDMA increased above normal (&gt; 14 mcg\/dL) in 17 of 21 cats a mean of 17 months (range, 1.5\u201348 months) before sCr exceeded its reference range (&gt; 2.1 mg\/dL) (<strong>Figure 2<\/strong>).<sup>20<\/sup><\/p>\n<div id=\"attachment_9177\" style=\"width: 419px\" class=\"wp-caption aligncenter\"><a href=\"https:\/\/todaysveterinarypractice.com\/wp-content\/uploads\/sites\/4\/2016\/02\/T1603F05Fig02.jpg\"><img decoding=\"async\" aria-describedby=\"caption-attachment-9177\" class=\"wp-image-9177 size-figure_img\" src=\"https:\/\/todaysveterinarypractice.com\/wp-content\/uploads\/sites\/4\/2016\/02\/T1603F05Fig02-409x300.jpg\" alt=\"FIGURE 2. Note that, although the laboratory\u2019s normal reference interval for sCr was &lt; 2.1 mg\/dL, a tighter reference interval for sCr (eg, &lt; 1.6 mg\/dL) would have decreased the 8 month disparity.<\/p><\/div>\n<p><strong>Dogs.<\/strong> In a study involving dogs that developed CKD, SDMA increased above normal (&gt; 14 mcg\/dL) a mean of 17 months (range, 11\u201326 months) before sCr exceeded its reference interval (&gt; 1.8 mg\/dL).<sup>21<\/sup><\/p>\n<p>In male dogs with X-linked hereditary nephropathy, using a single cutoff value for serum SDMA identified, on average, a less than 20% decrease in GFR as measured by iohexol plasma clearance.<sup>22<\/sup> In addition, with use of a single cutoff value for SDMA, reductions in GFR were detected earlier compared with a single sCr cutoff value or sCr trends over time.<sup>22<\/sup><\/p>\n<h3>Role of Lean Body Mass<\/h3>\n<p>Another potential advantage to monitoring serum SDMA concentrations for early diagnosis of CKD compared with monitoring sCr is that changes in lean body mass in dogs and cats do not appear to influence SDMA.<sup>23,24<\/sup><\/p>\n<h3>Role of Dietary Arginine<\/h3>\n<p>Although the effects of dietary arginine are as yet unknown, serum concentrations of SDMA are not correlated to serum arginine concentrations in dogs and cats. In humans, dietary arginine has no effect on serum SDMA concentrations.<sup>25<\/sup><\/p>\n<h3>Prognostic Value<\/h3>\n<p>Preliminary results suggest that serum SDMA: creatinine ratios may have prognostic value in dogs and cats with CKD (as long as the SDMA value is &gt; 14 mcg\/dL); ratios greater than 10 were associated with mortality within 1 year.<sup>26<\/sup><\/p>\n<div class=\"orange-box\">\n<h2>Monitoring &amp; Treatment Considerations<\/h2>\n<p>Dogs and cats with IRIS stage 1 CKD may not progress to stage 2; however, they are at risk for progression of kidney disease. Dogs and cats with stage 1 CKD warrant increased monitoring in order to assess the stability or progression of their kidney disease.<\/p>\n<p>There are no prospective, controlled clinical trials that demonstrate efficacy of any renoprotective treatments for stage 1 CKD. Renoprotective treatments, such as dietary modification and enteric phosphate binders, are typically initiated in iris stage 2 CKD.<\/p>\n<p>In patients with IRIS stage 2 CKD who have low body condition scores and SDMA concentrations of 25 mcg\/dl or greater, the sCR may have led to an underestimation of renal function; IRIS stage 3 CKD treatment recommendations may need to be considered.<\/p>\n<p>Similarly, in patients with IRIS stage 3 CKD who have low body condition scores and SDMA concentrations of 40 mcg\/dl or greater, the sCr may have led to an underestimation of renal function; iris stage 4 CKD treatment recommendations may need to be considered.<\/p>\n<\/div>\n<h2>Diagnostic Approach to CKD<\/h2>\n<h3>Serum Creatinine &amp; SDMA<\/h3>\n<p>On the basis of the preceding studies, it appears that compared with sCr, SDMA is a more sensitive renal function biomarker, and can facilitate early diagnosis and treatment of CKD.<\/p>\n<p>A persistent elevation in SDMA (&gt; 14 mcg\/dL), accompanied by an isosthenuric USG, even if sCr levels are less than 1.4 mg\/dL or 1.6 mg\/dL in dogs and cats, respectively, indicates reduced renal function and IRIS stage 1 CKD. Similar to sCr, SDMA needs to be interpreted in light of hydration status and physical examination findings. For example, a dehydrated dog or cat with a SDMA concentration of 16 mcg\/dL and a USG of &gt; 1.040 likely has prerenal azotemia. In contrast to sCr, SDMA should not be affected by changes in muscle mass.<\/p>\n<p>Dogs and cats with borderline sCr and\/or SDMA concentrations should be retested:<\/p>\n<ul>\n<li>Two weeks after initial testing to confirm the initial value<\/li>\n<li>Then approximately every 3 months to assess renal function stability.<\/li>\n<\/ul>\n<h3>Additional Diagnostics<\/h3>\n<p>Additional tests that may help further characterize potential renal disease and associated complications are listed in the <strong>Table<\/strong>. Bacterial pyelonephritis, partial or complete ureteral obstruction, hypertension, and proteinuria are all potentially treatable causes or complications of CKD.<\/p>\n<p><a href=\"https:\/\/todaysveterinarypractice.com\/wp-content\/uploads\/sites\/4\/2016\/02\/T1603F04Table.jpg\"><img decoding=\"async\" class=\"aligncenter wp-image-9174 \" src=\"https:\/\/todaysveterinarypractice.com\/wp-content\/uploads\/sites\/4\/2016\/02\/T1603F04Table.jpg\" alt=\"T1603F04Table\" width=\"316\" height=\"234\" srcset=\"https:\/\/navc.sitepreview.app\/todaysveterinarypractice.com\/wp-content\/uploads\/sites\/4\/2016\/02\/T1603F04Table.jpg 809w, https:\/\/navc.sitepreview.app\/todaysveterinarypractice.com\/wp-content\/uploads\/sites\/4\/2016\/02\/T1603F04Table-300x222.jpg 300w, https:\/\/navc.sitepreview.app\/todaysveterinarypractice.com\/wp-content\/uploads\/sites\/4\/2016\/02\/T1603F04Table-768x570.jpg 768w\" sizes=\"(max-width: 316px) 100vw, 316px\" \/><\/a><\/p>\n<h2>In Summary<\/h2>\n<p>Incorporating evaluation of serum SDMA along with closer monitoring of sCr may facilitate early diagnosis of CKD in dogs and cats. In general, diagnostic results that indicate increased monitoring include:<\/p>\n<ul>\n<li>sCr concentrations of <strong>1.4 mg\/dL or greater in dogs<\/strong> or <strong>1.6 mg\/dL or greater in cats<\/strong><\/li>\n<li>Serum SDMA concentrations <strong>greater than 14 mcg\/dL<\/strong>.<\/li>\n<\/ul>\n<p>Longitudinal assessment of these measures almost always provides better data than one-time evaluations. No single laboratory test is perfect; trending laboratory data, with the same test method, tends to improve diagnostic sensitivity.<\/p>\n<div class=\"orange-box\">\n<h2>SDMA Diagnostics in Practice<\/h2>\n<p>Serum SDMA concentration is now included in all routine IDEXX Reference Laboratories (idexx.com) serum biochemistry panels, with concentrations greater than 14 mcg\/dL considered abnormal in both dogs and cats.<\/p>\n<p>Similar to sCr, results from SDMA analysis must always be interpreted in light of patient USG and physical examination findings in order to rule out volume-responsive and postrenal causes of azotemia.
