Esophageal and gastric foreign bodies are commonly encountered in small animal practice. Endoscopy is the treatment of choice for esophageal and gastric foreign bodies, as it can be used to remove a variety of foreign objects with little risk to the patient. When compared with surgery, endoscopy offers clients a less invasive and less expensive option.<\/p>\n<p>However, before pursuing endoscopic removal of foreign bodies, carefully consider the following factors:<\/p>\n<ul>\n<li>Feasibility of removal<\/li>\n<li>Necessity of removal<\/li>\n<li>Urgency of removal<\/li>\n<li>Potential risks and complications.<\/li>\n<\/ul>\n<h2>How to Identify Foreign Bodies<\/h2>\n<p>&nbsp;<\/p>\n<p>The presentation of a pet with a GI foreign body varies depending on location, size, and configuration of the object as well as the degree of obstruction.<\/p>\n<h3><strong>Presentation<\/strong><\/h3>\n<p>Many patients with esophageal foreign bodies have a peracute presentation, although a chronic presentation is possible with partial obstruction. A patient with a freely movable gastric foreign body might present with intermittent vomiting, whereas a large or obstructive foreign body is more likely to cause frequent vomiting, poor appetite, and abdominal pain.<\/p>\n<p>Clinical signs of esophageal and gastric foreign bodies are listed in <strong>Table 1<\/strong>. However, clinical signs tend to be variable and may seem incongruous with the foreign body.<\/p>\n<p><a href=\"https:\/\/todaysveterinarypractice.com\/wp-content\/uploads\/sites\/4\/2015\/11\/T1511C08_Table01.jpg\"><img fetchpriority=\"high\" decoding=\"async\" class=\"size-medium wp-image-6649 aligncenter\" src=\"https:\/\/todaysveterinarypractice.com\/wp-content\/uploads\/sites\/4\/2015\/11\/T1511C08_Table01-300x289.jpg\" alt=\"T1511C08_Table01\" width=\"300\" height=\"289\" srcset=\"https:\/\/navc.sitepreview.app\/todaysveterinarypractice.com\/wp-content\/uploads\/sites\/4\/2015\/11\/T1511C08_Table01-300x289.jpg 300w, https:\/\/navc.sitepreview.app\/todaysveterinarypractice.com\/wp-content\/uploads\/sites\/4\/2015\/11\/T1511C08_Table01.jpg 622w\" sizes=\"(max-width: 300px) 100vw, 300px\" \/><\/a><\/p>\n<h3><strong>Radiography<\/strong><\/h3>\n<p>Survey radiographs of the chest and abdomen are indicated if a:<\/p>\n<ul>\n<li>Foreign body is suspected based on supportive clinical signs<\/li>\n<li>Client witnessed the pet ingest a foreign object<\/li>\n<li>Patient presents with fever or signs of shock indicative of luminal perforation (ie, esophageal, gastric, intestinal).<\/li>\n<\/ul>\n<p>When planning and evaluating radiographs:<\/p>\n<ul>\n<li>Include the neck in lateral projections if an esophageal foreign body is suspected (<strong>Figure 1<\/strong>)<\/li>\n<li>Characterize the foreign body; noting its location, size, and configuration<\/li>\n<li>Note whether pneumomediastinum or pleural effusion\u2014suggestive of esophageal perforation\u2014is present<\/li>\n<li>Remember that not all commonly ingested foreign objects are radiopaque (eg, plastic, wood).<\/li>\n<\/ul>\n<div id=\"attachment_6642\" style=\"width: 310px\" class=\"wp-caption aligncenter\"><a href=\"https:\/\/todaysveterinarypractice.com\/wp-content\/uploads\/sites\/4\/2015\/11\/T1511C08_Fig01.jpg\"><img decoding=\"async\" aria-describedby=\"caption-attachment-6642\" class=\"wp-image-6642 size-medium\" src=\"https:\/\/todaysveterinarypractice.com\/wp-content\/uploads\/sites\/4\/2015\/11\/T1511C08_Fig01-300x222.jpg\" alt=\"T1511C08_Fig01\" width=\"300\" height=\"222\" srcset=\"https:\/\/navc.sitepreview.app\/todaysveterinarypractice.com\/wp-content\/uploads\/sites\/4\/2015\/11\/T1511C08_Fig01-300x222.jpg 300w, https:\/\/navc.sitepreview.app\/todaysveterinarypractice.com\/wp-content\/uploads\/sites\/4\/2015\/11\/T1511C08_Fig01-768x567.jpg 768w, https:\/\/navc.sitepreview.app\/todaysveterinarypractice.com\/wp-content\/uploads\/sites\/4\/2015\/11\/T1511C08_Fig01.jpg 812w\" sizes=\"(max-width: 300px) 100vw, 300px\" \/><\/a><p id=\"caption-attachment-6642\" class=\"wp-caption-text\">FIGURE 1. Lateral thoracic radiograph of an esophageal foreign body (blue arrow) in a dog; note the esophageal dilation proximal to the foreign body (red arrow).<\/p><\/div>\n<h3><strong>Ultrasonography<\/strong><\/h3>\n<p>In practices with an ultrasound machine and an experienced ultrasonographer (ideally board certified), this imaging modality can be used\u2014as an initial diagnostic study or adjunct to radiography\u2014to evaluate patients for nonesophageal GI foreign bodies.<\/p>\n<p>Ultrasound has been shown to have a higher sensitivity than radiographs for identification of GI foreign material,<sup>1<\/sup> but is a more expensive, user-dependent modality that may not be readily available on an emergency basis. In addition, gastric ultrasound can be challenging when food and\/or gas, which can obscure the view of potential foreign material, is present. Further, it may be difficult to determine whether the material present is normal ingesta or a foreign body.<\/p>\n<p>However, additional benefits of ultrasonography include identification of abdominal lymphadenopathy, peritoneal fluid, and abnormal intestinal wall layering.<\/p>\n<h3><strong>Further Imaging Studies<\/strong><\/h3>\n<p>If a gastric foreign body is still suspected despite normal radiography and ultrasonography studies, alternative imaging studies can be considered. Negative contrast gastrograms highlight radiolucent foreign bodies, while positive contrast agents, such as barium, outline radiopaque objects in the stomach (<strong>Figure 2<\/strong>).<\/p>\n<div id=\"attachment_6643\" style=\"width: 248px\" class=\"wp-caption aligncenter\"><a href=\"https:\/\/todaysveterinarypractice.com\/wp-content\/uploads\/sites\/4\/2015\/11\/T1511C08_Fig02.jpg\"><img decoding=\"async\" aria-describedby=\"caption-attachment-6643\" class=\"size-medium wp-image-6643\" src=\"https:\/\/todaysveterinarypractice.com\/wp-content\/uploads\/sites\/4\/2015\/11\/T1511C08_Fig02-238x300.jpg\" alt=\"Figure 2. Ventrodorsal view of a barium study in a cat; the barium highlights a tubular foreign body (circled) in the stomach.<\/p><\/div>\n<h2>When to Pursue Endoscopy<\/h2>\n<p>Many foreign objects pass through the GI tract with little consequence; however, knowing which objects will take a benign course is difficult. Therefore, recommending a conservative \u201cwait and see\u201d approach is best reserved for pets that do not have significant clinical signs and have ingested small, round, or blunt foreign objects.<\/p>\n<p>Endoscopic removal should be pursued for:<\/p>\n<ul>\n<li>Sharp objects, such as needles; although some needles pass uneventfully, others may puncture the GI tract, resulting in peritonitis<\/li>\n<li>Toxic materials, which commonly include lead, zinc (pennies minted after 1982), and small disk batteries containing alkali (eg, watch batteries) (<strong>Figure 3<\/strong>)<\/li>\n<li>Objects that have been retained for more than 2 to 3 weeks.<\/li>\n<\/ul>\n<p>Objects that are very difficult or impossible to remove from the stomach endoscopically include corn cobs, large rocks, large balls, polyurethane glue, and heavy objects.<\/p>\n<div id=\"attachment_6644\" style=\"width: 310px\" class=\"wp-caption aligncenter\"><a href=\"https:\/\/todaysveterinarypractice.com\/wp-content\/uploads\/sites\/4\/2015\/11\/T1511C08_Fig03.jpg\"><img loading=\"lazy\" decoding=\"async\" aria-describedby=\"caption-attachment-6644\" class=\"size-medium wp-image-6644\" src=\"https:\/\/todaysveterinarypractice.com\/wp-content\/uploads\/sites\/4\/2015\/11\/T1511C08_Fig03-300x222.jpg\" alt=\"Figure 3. Lateral abdominal radiograph of gastric foreign bodies; multiple stacked coins can be seen in the antrum of the stomach.<\/p><\/div>\n<h2>What Instrumentation Is Needed<\/h2>\n<h3><strong>Retrieval Instruments<\/strong><\/h3>\n<p>A variety of foreign body retrieval instruments are available (<strong>Figure 4<\/strong>). At minimum, those needed for foreign body retrieval include:<\/p>\n<ul>\n<li>Long-arm 2-prong grasping forceps<\/li>\n<li>Snare loop.<\/li>\n<\/ul>\n<p><strong>Table 2<\/strong> provides a complete list of commonly used foreign body retrieval instruments. Note that retrieval forceps may be limited by the size of the working channel; large instruments require a 2.8-mm working channel. Do not use biopsy forceps because the foreign body may damage or blunt the instrument. Overtubes are designed to pass over the endoscope; they should be 2 mm larger than the endoscope and 50 to 60 cm in length. Foreign body hoods are rubber latex shields that fit over the distal aspect of most gastroscopes.<\/p>\n<h2>Esophageal Foreign Body Retrieval<\/h2>\n<h3><strong>Prior to Procedure<\/strong><\/h3>\n<p>Esophageal foreign bodies should be removed promptly because complications are more likely when the foreign body has prolonged contact with the esophageal mucosa. However, take time to pursue the following steps prior to retrieving foreign bodies:<\/p>\n<ol>\n<li>Perform routine preanesthetic blood analysis and complete physical examination to evaluate systemic health, and tailor the anesthetic plan, when possible.<\/li>\n<li>Review radiographs with particular attention to the location and quantity of objects; foreign objects are often seen at areas of anatomic narrowing, including the:\n<ul>\n<li>Upper esophageal sphincter (UES)<\/li>\n<li>Thoracic inlet<\/li>\n<li>Heart base<\/li>\n<li>Distal esophagus proximal to the gastroesophageal junction<\/li>\n<\/ul>\n<\/li>\n<li>Discuss risks and possible complications with the client.<\/li>\n<\/ol>\n<h3><strong>Endoscopic Retrieval<\/strong><\/h3>\n<h3><\/h3>\n<ol>\n<li>Apply <a href=\"https:\/\/todaysveterinarypractice.com\/endoscopy-essentials-upper-gastrointestinal-endoscopy-techniques-part-2\/\">principles of general endoscopic technique<\/a>, including:\n<ul>\n<li>Placing the patient in left lateral recumbency<\/li>\n<li>Ensuring the endotracheal tube cuff is well inflated.<\/li>\n<\/ul>\n<\/li>\n<li>Distend the esophageal lumen with air to obtain good visualization, and designate an assistant to monitor the degree of gastric distension. Acute respiratory decompensation can result from insufflation into a perforated esophagus or severe overdistension.<\/li>\n<li>Once the object is encountered, thoroughly inspect it and ask these questions:\n<ul>\n<li><span style=\"color: #000000\">Is the object embedded in the esophageal mucosa?<\/span><\/li>\n<li>Are there ridges or grooves that can be grasped easily?<\/li>\n<li>Is the position amenable to extraction?<\/li>\n<li>Are there sharp edges that could damage the mucosa during extraction?<\/li>\n<\/ul>\n<\/li>\n<li>Select a retrieval instrument based on the appearance of the foreign body and grasp the object firmly, seating the retrieval instrument onto the object as much as possible.<\/li>\n<li>Withdraw the instrument to the tip of the endoscope; then remove the object and scope together, slowly and with minimal force.<\/li>\n<li>If the object needs to be adjusted to facilitate grasping it, avoid using the endoscope, which can damage its tip. Instead, use an overtube to push or manipulate the object.<\/li>\n<li>During removal, if the:\n<ul>\n<li>Object has a sharp end, when possible position the sharp end either caudally or within the retrieval instrument (parallel to the lumen) to prevent damage as the object is retrieved<\/li>\n<li>Sharp end cannot be retrieved as described, encase the object in an overtube before removal<\/li>\n<li>Object gets caught at the UES, endotracheal cuff deflation may facilitate extraction.<\/li>\n<\/ul>\n<\/li>\n<\/ol>\n<h3><strong>Additional Retrieval Techniques<\/strong><\/h3>\n<p>The majority of esophageal foreign bodies can be removed quickly and without complication. However, when objects cannot be retrieved easily, the following techniques can be considered.<\/p>\n<p><strong>Foley Catheter Technique.<\/strong> If object configuration renders grasping it impossible, removal using a Foley catheter can be attempted:<\/p>\n<ol>\n<li>Advance the Foley catheter past the foreign body, employing a guidewire if needed<\/li>\n<li>Inflate the cuff, and gently and slowly withdraw the catheter.<\/li>\n<\/ol>\n<p><strong>Techniques for Sharp Objects.<\/strong> If foreign bodies are sharp or have become embedded in the esophageal mucosa, the object must first be dislodged before grasping and removing it, which can be particularly challenging.<\/p>\n<ol>\n<li>Techniques that may facilitate removal include full air distension of the esophagus, balloon dilation of the esophagus proximal to the object, and short interchangeable push and pull movements.<\/li>\n<li>If the object is stuck within the esophagus and in reach of a rigid grasping forceps, attempt removal with a gentle twisting motion. Do <em>not<\/em> use this technique with fishhooks.<\/li>\n<li>Dislodge fishhooks from the esophageal mucosa by grasping the stem and deflecting the tip of the endoscope away from the wall (<strong>Figure 5<\/strong>). Ensure that the fishhook\u2019s point is directed caudally, whenever possible, during extraction.<\/li>\n<\/ol>\n<div id=\"attachment_6646\" style=\"width: 310px\" class=\"wp-caption aligncenter\"><a href=\"https:\/\/todaysveterinarypractice.com\/wp-content\/uploads\/sites\/4\/2015\/11\/T1511C08_Fig05.jpg\"><img loading=\"lazy\" decoding=\"async\" aria-describedby=\"caption-attachment-6646\" class=\"size-medium wp-image-6646\" src=\"https:\/\/todaysveterinarypractice.com\/wp-content\/uploads\/sites\/4\/2015\/11\/T1511C08_Fig05-300x300.jpg\" alt=\"Figure 5. Endoscopic image of a fishhook esophageal foreign body in a dog.<\/p><\/div>\n<h3><strong>Surgical Techniques<\/strong><\/h3>\n<p><strong>Thoracotomy.<\/strong> To avoid perforation, the endoscopic retrieval procedure should be aborted and a thoracotomy performed if:<\/p>\n<ul>\n<li>The object is well embedded<\/li>\n<li>Little to no progress is made using the previously described techniques<\/li>\n<li>The object cannot be pushed into the stomach for removal via laparotomy.<\/li>\n<\/ul>\n<p><strong>Gastrotomy.<\/strong> If the object is distal in the esophagus, it may be possible to perform a laparotomy to remove the object via a gastrotomy. If perforation is suspected, obtain follow-up thoracic radiographs.<\/p>\n<h3><strong>After Removal<\/strong><\/h3>\n<p>Once the object is removed:<\/p>\n<ol>\n<li>Ensure that no additional foreign objects are present.<\/li>\n<li>Evaluate the mucosal surface thoroughly for damage (<strong>Figure 6<\/strong>). Chronic mucosal injury may weaken the esophageal wall, increasing the risk for iatrogenic tearing during foreign body extraction.<\/li>\n<li>Consider treatment of esophagitis (see <strong>Treatment of Esophagitis &amp; Severe Esophageal Injuries<\/strong>) when the mucosa appears erythematous, eroded, or ulcerated.<\/li>\n<li>Offer soft food to the patient for 18 to 24 hours after the procedure.<\/li>\n<li>Note that exuberant post-procedure healing may lead to esophageal stricture.<\/li>\n<\/ol>\n<div id=\"attachment_6647\" style=\"width: 310px\" class=\"wp-caption aligncenter\"><a href=\"https:\/\/todaysveterinarypractice.com\/wp-content\/uploads\/sites\/4\/2015\/11\/T1511C08_Fig06.jpg\"><img loading=\"lazy\" decoding=\"async\" aria-describedby=\"caption-attachment-6647\" class=\"size-medium wp-image-6647\" src=\"https:\/\/todaysveterinarypractice.com\/wp-content\/uploads\/sites\/4\/2015\/11\/T1511C08_Fig06-300x150.jpg\" alt=\"Figure 6. Endoscopic image of a rawhide esophageal foreign body in a dog being removed with a snare retrieval instrument (A); note the esophageal pressure necrosis and mucosal erosion following extraction (B).<\/p><\/div>\n<div class=\"orange-box\">\n<h2><strong>Treatment of Esophagitis and Severe Esophageal Injuries<\/strong><\/h2>\n<p>Typical treatment options for esophagitis include:<\/p>\n<ul>\n<li>Carafate slurry<\/li>\n<li>Proton pump inhibitors<\/li>\n<li>H-2 receptor antagonists.<\/li>\n<\/ul>\n<p>Treatment options for severe esophageal injury secondary to esophageal foreign body removal include:<\/p>\n<ul>\n<li>Antimicrobials: For deep ulceration or perforation<\/li>\n<li>Metoclopramide: To increase lower esophageal sphincter (LES) pressure<\/li>\n<li>Corticosteroids: If there is no evidence of infection, a short tapering course of prednisone may reduce the fibroblastic response, decreasing risk for stricture formation.<\/li>\n<\/ul>\n<p>With severe esophageal injury, consider placement of a gastric tube to allow esophageal rest while providing adequate nutrition.<\/p>\n<p>For more information on medical therapy for the GI tract, read <strong>Symptomatic Management of Primary Acute Gastroenteritis.<br \/>\n<\/strong><\/p>\n<\/div>\n<h2>Gastric Foreign Body Retrieval<\/h2>\n<h3><strong>Prior to Procedure<\/strong><\/h3>\n<ol>\n<li>Perform routine preanesthetic blood analysis and complete physical examination to evaluate systemic health, and tailor the anesthetic plan, when possible.<\/li>\n<li>Ideally, fast the patient for 6 to 8 hours prior to gastroscopy, which reduces the risk of aspiration and allows better visualization of the foreign body. If the foreign body must be removed on an emergent basis:\n<ul>\n<li>Pass a stomach tube to lavage the stomach and remove gastric contents, which may improve visibility and aid in foreign body identification (<strong>Figure 7<\/strong>)<\/li>\n<li>Alternatively, rotate the patient to right lateral or ventral recumbency during the procedure, which can also aid in locating the foreign body or making it more accessible.<\/li>\n<\/ul>\n<\/li>\n<li>Review all radiographs taken within the previous hour and examine the entire length of the GI tract for evidence of additional foreign bodies, noting the number and general location of the objects.\n<ul>\n<li>Timeliness of radiography is crucial because, if gastric motility is normal, an object may move from the stomach into the small intestine between the time of the last radiograph and the procedure.<\/li>\n<li>Therefore, a common, but avoidable, mistake during endoscopy is failure to locate a foreign body in the stomach because it has moved into the small intestine.<\/li>\n<li>Remember that endoscopic retrieval can only be reliably employed for objects in the esophagus and stomach; it is imperative to ensure there are no objects within the intestinal tract.<\/li>\n<\/ul>\n<\/li>\n<\/ol>\n<div id=\"attachment_6648\" style=\"width: 310px\" class=\"wp-caption aligncenter\"><a href=\"https:\/\/todaysveterinarypractice.com\/wp-content\/uploads\/sites\/4\/2015\/11\/T1511C08_Fig07.jpg\"><img loading=\"lazy\" decoding=\"async\" aria-describedby=\"caption-attachment-6648\" class=\"size-medium wp-image-6648\" src=\"https:\/\/todaysveterinarypractice.com\/wp-content\/uploads\/sites\/4\/2015\/11\/T1511C08_Fig07-300x222.jpg\" alt=\"Figure 7. Endoscopic image of 3 coins lodged in the stomach of a dog<\/p><\/div>\n<h3><strong>Endoscopic Retrieval<\/strong><\/h3>\n<ol>\n<li>Follow steps 1 and 2 from <strong>Esophageal Foreign Body Retrieval: Endoscopic Retrieval.<\/strong><\/li>\n<li>Once the object has been found and characterized, select an instrument and advance it into the working channel.<\/li>\n<li>Grasp the object firmly, then withdraw it to the endoscope tip, attempting to create favorable alignment between the object and endoscope to minimize resistance and facilitate smooth extraction through the LES and UES.<\/li>\n<li>Retract the endoscope and object as a unit, slowly and without force.<\/li>\n<li>During removal consider:\n<ul>\n<li>Deflecting the endoscope tip 30\u00b0 to 40\u00b0 to mimic the natural angle of the esophagus and stomach, if the object is difficult to remove through the LES<\/li>\n<li>Removing air from the stomach to create a less acute angle with the esophagus<\/li>\n<li>Using an overtube to help distend the LES enough for the object to pass through.<\/li>\n<li>Leaving an overtube in the stomach to facilitate more rapid removal of numerous foreign bodies because the operator can quickly slide the endoscope in and out of the patient.<\/li>\n<\/ul>\n<\/li>\n<\/ol>\n<h3><strong>Additional Technique<\/strong><\/h3>\n<p>If the object is circular (e.g., a prong collar) and difficult to grasp, a suture technique may be helpful. This technique results in placement of a loop of suture through the foreign body, allowing the operator to remove the object by pulling both ends of the suture with gentle traction.<\/p>\n<ol>\n<li>Retract the grasping forceps through the working channel of the endoscope to grasp one end of a monofilament suture, while holding the free end of the suture outside the patient.<\/li>\n<li>Advance the forceps back into the tip of the endoscope and then the endoscope to the foreign body.<\/li>\n<li>Once at the desired location, advance the suture around or through the object and release it; then re-grasp the suture on the other side of the object and withdraw the endoscope.<\/li>\n<\/ol>\n<h3><strong>After Removal<\/strong><\/h3>\n<ol>\n<li>Once an object is removed, inspect the stomach thoroughly for mucosal disease or additional foreign material.<\/li>\n<li>If the foreign body is chronic (suggestive of a motility disorder) or chronic GI disease precipitating ingestion of the foreign body is suspected, <a href=\"https:\/\/todaysveterinarypractice.com\/endoscopy-essentials-part-1-overview-of-upper-gastrointestinal-endoscopy-2\/\">perform a complete upper GI endoscopy<\/a>.<\/li>\n<li>Follow-up treatment is not usually required and the patient may be offered food 8 to 12 hours after the procedure.<\/li>\n<li>Treat patients with visible erosions or ulcerations as outlined earlier for esophagitis.<br \/>\n<hr \/>\n<\/li>\n<\/ol>\n<p>GI = gastrointestinal; LES = lower esophageal sphincter; UES = upper esophageal sphincter<\/p>\n<hr \/>\n<h3><strong>Suggested Readings<\/strong><\/h3>\n<p>Gualtieri M.
