However, those big eyes\u2014with their endearing qualities\u2014can be very problematic for these breeds.<\/p>\n<p style=\"text-align: left\">The majority of small dogs, especially the brachycephalic breeds, have palpebral fissures that are too long to provide adequate coverage of the prominent globes because of macroblepharon (an abnormally large eyelid opening).<sup>1<\/sup> In many cases, even if these dogs can complete an effective blink, their lids cannot close completely (lagophthalmos), or frequently, enough to lubricate and protect the ocular surface.<\/p>\n<hr \/>\n<p>Each year, the NAVC Institute takes place in Orlando, Florida, and top specialists in select areas of veterinary medicine provide hands-on, one-on-one continuing education to Institute attendees.The NAVC and <em>Today\u2019s Veterinary Practice<\/em> have partnered together to present the <strong>Practical Techniques from the NAVC Institute<\/strong> column, which includes material from sessions presented at the NAVC Institute 2014.<\/p>\n<hr \/>\n<p style=\"text-align: left\">Inability to effectively blink predisposes the eyes to chronic exposure, which can result in a wide variety of corneal problems, including, but not limited to:<br \/>\n\u2022 Corneal ulcerations and erosions<br \/>\n\u2022 Vascular keratitis<br \/>\n\u2022 Pigmentary keratitis<br \/>\n\u2022 Corneal fibrosis.<\/p>\n<p style=\"text-align: left\">All of these problems, which may be combined with concurrent keratoconjunctivitis sicca, subsequently affect corneal clarity, and can result in pain and decreased vision.<sup>2,3<\/sup><\/p>\n<h2 style=\"text-align: left\">Components of Brachycephalic Ocular Syndrome<\/h2>\n<p style=\"text-align: left\">The \u201cso-called\u201d <em>brachycephalic ocular condition<\/em> (<strong>Figure 1<\/strong>), similar to <em>brachycephalic respiratory condition<\/em>, can be considered a syndrome that\u2014in addition to macroblepharon and lagophthalmos\u2014 may include a variety of other factors or clinical signs.<sup>1-5<\/sup> Brachycephalic breeds often have:<\/p>\n<ul style=\"text-align: left\">\n<li><strong>Medial canthal entropion,<\/strong> in which the lower eyelid, adjacent to the medial canthus, rolls inward and covers up, or occludes, the lower nasolacrimal puncta<sup>2,5,6<\/sup><\/li>\n<li><strong>Epiphora and tear staining<\/strong> due to inappropriate tear fluid drainage that, sometimes, are significant enough to cause moist dermatitis ventral to the eye<\/li>\n<li><strong>Trichiasis<\/strong>, or hairs that deviate toward the cornea from the medial caruncle or nasal skin folds adjacent to (sometimes contacting) the cornea;<sup>6<\/sup> this hair chronically irritates the ocular surface\u2014even if the patient does not demonstrate discomfort\u2014and can contribute to keratitis and ocular melanosis.<sup>2,3,7<\/sup> The medial caruncle is a raised nodule of conjunctiva overlying glandular tissue that sits in the corner of the eye adjacent to the medial canthus<\/li>\n<li><strong>Reduced corneal sensitivity<\/strong> compared to mesocephalic and dolichocephalic breeds;<sup>2-4<\/sup> thus, while afflicted dogs may not exhibit acute or severe corneal pain, diminished innervation can contribute to delayed recognition of the problem and ongoing damage that leads to impaired corneal wound healing<\/li>\n<li><strong>Quantitative or qualitative tear deficiencies<\/strong>, which can further exacerbate corneal injury<\/li>\n<li><strong>Shallow orbits<\/strong>, which offer considerably less protection of the globe than the deeper orbits of their mesocephalic or dolichocephalic counterparts.<sup>2-4<\/sup> This orbital anatomy, especially when combined with excessively long palpebral fissures, greatly increases potential for traumatic proptosis, a globe-threatening condition in which the eyelid margins become entrapped behind the equator of the globe, making it impossible for the patient to blink and protect the anterior structures of the globe. Damage to the optic nerve and extraocular muscles is common when the initial injury occurs, and, if the condition is not promptly repaired, exposure, corneal ulcerations, and subsequent globe rupture can occur.<\/li>\n<\/ul>\n<p style=\"text-align: left\"><a href=\"https:\/\/todaysveterinarypractice.com\/wp-content\/uploads\/sites\/4\/2015\/03\/fig1A.png\"><img fetchpriority=\"high\" decoding=\"async\" class=\"size-medium wp-image-7580 aligncenter\" src=\"https:\/\/todaysveterinarypractice.com\/wp-content\/uploads\/sites\/4\/2015\/03\/fig1A-300x255.png\" alt=\"fig1A\" width=\"300\" height=\"255\" \/><\/a><\/p>\n<p style=\"text-align: left\"><a href=\"https:\/\/todaysveterinarypractice.com\/wp-content\/uploads\/sites\/4\/2015\/03\/fig1B.png\"><img decoding=\"async\" class=\"wp-image-7581 aligncenter\" src=\"https:\/\/todaysveterinarypractice.com\/wp-content\/uploads\/sites\/4\/2015\/03\/fig1B-300x212.png\" alt=\"fig1B\" width=\"361\" height=\"255\" \/><\/a><\/p>\n<p style=\"text-align: left\">FIGURE 1. Note the prominent globe (emphasize by the shallow orbit), medial canthal entropion and trichiasis, and tear staining from epiphora (B).<\/p>\n<h2 style=\"text-align: left\">Surgical Approach<\/h2>\n<p style=\"text-align: left\">In a patient with severe lagophthalmos and resultant ocular disease, a procedure known as <em>medial canthoplasty<\/em> can help considerably (<strong>Figure 2<\/strong>).<sup>1-8<\/sup> Although it does not correct all anatomic and physiologic features that threaten globe health and security, it can be very effective at reducing exposure and frictional irritation that can lead to keratitis and pigmentation.<\/p>\n<p style=\"line-height: 1.5;text-align: left\">This procedure corrects the characteristics of brachycephalic ocular syndrome by:<br \/>\n\u2022 Reducing the length of both the upper and lower eyelids and overall length of the palpebral fissure<br \/>\n\u2022 Everting the medial inferior entropion<br \/>\n\u2022 Relieving functional obstruction of thenasolacrimal apparatus<br \/>\n\u2022 Removing the hairy medial caruncle and any additional medially deviated trichiasis.<\/p>\n<p style=\"line-height: 1.5;text-align: left\">By improving globe coverage, effectiveness of the blink mechanism may be enhanced, tear fluid may be better distributed across the ocular surface, and the risk of proptosis may be reduced.<\/p>\n<p style=\"line-height: 1.5;text-align: left\"><a href=\"https:\/\/todaysveterinarypractice.com\/wp-content\/uploads\/sites\/4\/2015\/03\/F01_Fig02A.png\"><img decoding=\"async\" class=\"wp-image-7582 aligncenter\" src=\"https:\/\/todaysveterinarypractice.com\/wp-content\/uploads\/sites\/4\/2015\/03\/F01_Fig02A-300x199.png\" alt=\"F01_Fig02A\" width=\"378\" height=\"251\" \/><\/a><br \/>\n<a href=\"https:\/\/todaysveterinarypractice.com\/wp-content\/uploads\/sites\/4\/2015\/03\/F01_Fig02B.png\"><img loading=\"lazy\" decoding=\"async\" class=\"wp-image-7583 aligncenter\" src=\"https:\/\/todaysveterinarypractice.com\/wp-content\/uploads\/sites\/4\/2015\/03\/F01_Fig02B-300x199.png\" alt=\"F01_Fig02B\" width=\"384\" height=\"255\" \/><\/a><\/p>\n<div id=\"attachment_7584\" style=\"width: 398px\" class=\"wp-caption aligncenter\"><a href=\"https:\/\/todaysveterinarypractice.com\/wp-content\/uploads\/sites\/4\/2015\/03\/F01_Fig02C.png\"><img loading=\"lazy\" decoding=\"async\" aria-describedby=\"caption-attachment-7584\" class=\"wp-image-7584\" src=\"https:\/\/todaysveterinarypractice.com\/wp-content\/uploads\/sites\/4\/2015\/03\/F01_Fig02C-300x253.png\" alt=\"F01_Fig02C\" width=\"388\" height=\"327\" \/><\/a><p id=\"caption-attachment-7584\" class=\"wp-caption-text\">FIGURE 2. A pug dog with brachycephalic ocular syndrome (A and B); another pug dog following medial canthoplasty (C).<\/p><\/div>\n<h2 style=\"line-height: 1.5;text-align: left\">Medial Canthoplasty: Step by Step<\/h2>\n<p style=\"line-height: 1.5;text-align: left\">Following careful client counselling and patient selection, the procedure is performed under general anesthesia. Patients should be suitable candidates for general anesthesia and clients should be prepared for the requisite postoperative care. Owners will be expected to maintain the patient in an Elizabethan collar until suture removal and to administer topical and oral medications diligently after surgery.<\/p>\n<p style=\"line-height: 1.5;text-align: left\"><strong>Preparation<\/strong><br \/>\n1. Under general anesthesia, clip the hair of both eyelids and cleanse and prepare the surgical site with betadine solution diluted in 0.9% saline to a concentration of 0.5%. Note that betadine is the only antiseptic that should be used near the eye; all others, including chlorhexidine, can cause corneal ulceration.<br \/>\n2. Carefully clean out the palpebral fornices with sterile cotton-tipped applicators and rinse the eyes and lids with saline or eye wash.<br \/>\n3. Use a caliper or other fine measuring device to measure the length of each eye\u2019s palpebral fissure and the upper and lower eyelids of both eyes (<strong>Figure 3)<\/strong>. A palpebral fissure length is usually considered excessive if it is longer than the corneal diameter by more than 6 to 8 mm.<\/p><\/div>\n<div class=\"orange-box\"><strong>Determining Eyelid Fissure Size<\/strong><br \/>\nWhen determining ideal eyelid fissure size, it is helpful to begin with an idea of what the individual dog&#8217;s ideal eyelid length would be, and then plan to make both palpebral fissures that same length.The average dog&#8217;s corneal diameter is approximately 14 to 16 mm, but the palpebral fissure length may range between 23 and 38 mm (even longer in some large breed dogs).3 Most dogs do well if their palpebral fissures are approximately 6 to 8 mm longer than their corneal diameter.However, this is merely a guideline. The perfect eyelid fissure size for an individual depends upon its anatomy and the desired cosmetic outcome. Blepharoplasty procedures involve a degree of aesthetic awareness and a good understanding of the required function of the eyelids.<\/div>\n<p>&nbsp;<\/p>\n<p style=\"line-height: 1.5;text-align: left\"><strong>Surgery<\/strong><br \/>\n1. Identify the upper and lower nasolacrimal puncta at the medial canthus (<strong>Figure 4<\/strong>), and cannulate these puncta and ducts to prevent inadvertent damage during surgery. Leave cannulas in place for the duration of the procedure.<\/p>\n<p style=\"line-height: 1.5;text-align: left\">Ideally, this procedure should be performed with the aid of magnification to ensure that tissue dissection and suture placement is exact.<\/p>\n<p style=\"line-height: 1.5;text-align: left\">Specific instruments and nasolacrimal cannulas may be purchased for this purpose; however, a large gauge suture, such as 2-0 blue or green monofilament, or a short small-gauge IV catheter (24- or 25-gauge) threaded into the duct may be employed, especially if the hub of the catheter is removed to prevent interference with tissue access.<\/p>\n<p style=\"line-height: 1.5;text-align: left\"><a href=\"https:\/\/todaysveterinarypractice.com\/wp-content\/uploads\/sites\/4\/2015\/03\/F01_Fig04A.png\"><img loading=\"lazy\" decoding=\"async\" class=\"size-full wp-image-7585 aligncenter\" src=\"https:\/\/todaysveterinarypractice.com\/wp-content\/uploads\/sites\/4\/2015\/03\/F01_Fig04A.png\" alt=\"F01_Fig04A\" width=\"280\" height=\"237\" \/><\/a><\/p>\n<div id=\"attachment_7586\" style=\"width: 290px\" class=\"wp-caption aligncenter\"><a href=\"https:\/\/todaysveterinarypractice.com\/wp-content\/uploads\/sites\/4\/2015\/03\/F01_Fig04B.png\"><img loading=\"lazy\" decoding=\"async\" aria-describedby=\"caption-attachment-7586\" class=\"wp-image-7586 size-full\" src=\"https:\/\/todaysveterinarypractice.com\/wp-content\/uploads\/sites\/4\/2015\/03\/F01_Fig04B.png\" alt=\"F01_Fig04B\" width=\"280\" height=\"237\" \/><\/a><p id=\"caption-attachment-7586\" class=\"wp-caption-text\">FIGURE 4. Identify the area of tissue to be resected (<strong>Figure 5<\/strong>) and make an incision with a No. 64 Beaver blade (beaver-visitec.com) in the skin around the medial canthus to delineate the portions of the eyelid margin, medial caruncle, and conjunctiva medial to the canthus to be removed. In most cases the incision will extend almost to the level of the nasolacrimal puncta, but this will vary on a case-by-case basis depending upon the position of the puncta and the overall length of the fissure. Undermine the section to be resected with small tenotomy or Westcott scissors, taking care to avoid the nasolacrimal apparatus (<strong>Figure 6<\/strong>). Close the surgical site in 2 layers:<\/p>\n<ul>\n<li>Appose the deep conjunctival layer with a small gauge absorbable suture, such as 6-0 Vicryl (ethicon.com), in a simple continuous pattern, with the knots buried away from the corneal surface (<strong>Figure 7<\/strong>).<\/li>\n<li>Repair the medial canthus with a figure-of-8 suture (<strong>Figure 8<\/strong>)<strong>,<\/strong> and close the remainder of skin with simple interrupted small gauge nonabsorbable sutures, such as 5-0 nylon (<strong>Figure 9<\/strong>).<\/li>\n<li>It may be helpful to place one partial thickness horizontal mattress tarsorrhaphy suture through the eyelid margin, adjacent to the medial canthal repair, to reduce tension as the site heals.\n<p style=\"line-height: 1.5;text-align: left\"><a href=\"https:\/\/todaysveterinarypractice.com\/wp-content\/uploads\/sites\/4\/2015\/03\/F01_Fig07A.png\"><img loading=\"lazy\" decoding=\"async\" class=\"size-full wp-image-7573 aligncenter\" src=\"https:\/\/todaysveterinarypractice.com\/wp-content\/uploads\/sites\/4\/2015\/03\/F01_Fig07A.png\" alt=\"F01_Fig07A\" width=\"280\" height=\"237\" \/><\/a><\/p>\n<div id=\"attachment_7574\" style=\"width: 290px\" class=\"wp-caption aligncenter\"><a href=\"https:\/\/todaysveterinarypractice.com\/wp-content\/uploads\/sites\/4\/2015\/03\/F01_Fig07B.png\"><img loading=\"lazy\" decoding=\"async\" aria-describedby=\"caption-attachment-7574\" class=\"wp-image-7574 size-full\" src=\"https:\/\/todaysveterinarypractice.com\/wp-content\/uploads\/sites\/4\/2015\/03\/F01_Fig07B.png\" alt=\"F01_Fig07B\" width=\"280\" height=\"237\" \/><\/a><p id=\"caption-attachment-7574\" class=\"wp-caption-text\">FIGURE 7. Warn the client that the initial appearance of the palpebral fissure will seem small, especially with a temporary tarsorrhaphy in place, but once the sutures are removed, the pet\u2019s appearance will be normal.<\/p>\n<h3 style=\"line-height: 1.5;text-align: left\"><strong>Considerations<\/strong><\/h3>\n<p style=\"line-height: 1.5;text-align: left\">While the medial canthoplasty procedure itself is relatively straightforward, several very important considerations require attention and thoughtfulness.<\/p>\n<p style=\"line-height: 1.5;text-align: left\">During surgery, it is critical to:<\/p>\n<ol>\n<li>Identify and avoid the nasolacrimal ducts during tissue excision and suturing<\/li>\n<li>Be meticulous during removal of the medial caruncular and conjunctival hair follicles<\/li>\n<li>Be precise during reapposition of the medial canthus.<\/li>\n<\/ol>\n<p>During the postoperative period, the patient must be treated appropriately.<sup>2,3<\/sup><\/p>\n<p style=\"line-height: 1.5;text-align: left\"><a href=\"https:\/\/todaysveterinarypractice.com\/wp-content\/uploads\/sites\/4\/2015\/03\/Screen-Shot-2015-03-25-at-5.57.39-PM.png\"><img loading=\"lazy\" decoding=\"async\" class=\"wp-image-1224 aligncenter\" src=\"https:\/\/todaysveterinarypractice.com\/wp-content\/uploads\/sites\/4\/2015\/03\/Screen-Shot-2015-03-25-at-5.57.39-PM.png\" alt=\"Brachycephalic Ocular Syndrome\" width=\"427\" height=\"248\" \/><\/a><\/p>\n<ol>\n<li>Patients should wear an Elizabethan collar to prevent self- trauma and premature removal of sutures. If the incision site<br \/>\nand eyelid structure break down, eyelid function and the globe will be compromised.<\/li>\n<li>Most patients benefit from topical broad-spectrum antibiotic therapy and a few days of systemic anti-inflammatory and, possibly, analgesic medications (<strong>Table<\/strong>).<\/li>\n<li>Sutures should be left in place for a full 14 days; earlier removal may cause incision failure.<\/li>\n<\/ol>\n<h2 style=\"text-align: left\"><strong>Summary<\/strong><\/h2>\n<p style=\"text-align: left\">In summary, brachycephalic\u00a0dogs often display ocular surface disease that can be correlated with their breed-related oculofacial conformation of elongated eyelids, medial canthal entropion, and caruncular trichiasis.
