Despite increased awareness of this fact, levels of serious psychological distress have increased over time.<sup>1<\/sup> There are many theories as to why veterinary professionals experience significant distress, some of which are supported by data, while others are anecdotal. Ultimately, mental health struggles are multifactorial, and being a member of the veterinary profession can be a contributing factor but not the sole\u00a0cause.<\/span><\/p>\n<p class=\"p1\"><span class=\"s1\">Just like any other profession, veterinary medicine has its own culture. This influences the way members of the profession behave and think about themselves. Unfortunately, because perfectionism, overwork, and self-sufficiency have been highly valued in the profession historically, some members of the profession have been reluctant to seek help for mental health struggles for fear of judgment or not fitting the cultural norm. By understanding the current cultural norms, especially those that influence help-seeking behaviors, we can begin to find ways to create change and move toward a healthier professional environment.<\/span><\/p>\n<h2 class=\"p2\">A Culture of Perfection<\/h2>\n<p class=\"p1\"><span class=\"s1\">Younger veterinarians (ages 18 to 34) have higher levels of serious psychological distress and burnout than <\/span>their older counterparts.<sup>1<\/sup> Exploring why this population<span class=\"s1\"> is at higher risk can help us create strategies that can improve the profession as a whole. One factor that has been more recently explored in the literature is the <\/span>effect of culture on the willingness of veterinary students<span class=\"s1\"> to seek help for mental health concerns. Tamara Hancock, DVM, MS, PhD, DACVP (Clinical), assistant teaching professor at the University of Missouri\u2019s College of Veterinary Medicine, has researched this phenomenon with Kerry Karaffa, PhD. Dr. Hancock describes the unique experience of veterinary students as \u201can artificial mashing together of a cohort experience. Everyone is doing the exact same thing on the exact same schedule. Everything feels like it\u2019s high <\/span>stakes.\u201d This contributes to the creation of a perfectionist<span class=\"s1\"> culture and public stigma that influence the likelihood a student will seek help. \u201cWe ask <\/span>students to be the best in everything to gain admission,\u201d<span class=\"s1\"> says Dr. Hancock, which creates an expectation of perfection before veterinary school ever begins. Ivan Zak, DVM, MBA, CEO of <a href=\"https:\/\/galaxyvets.com\/\" target=\"_blank\" rel=\"noopener\">Galaxy Vets<\/a>, notes that veterinary schools \u201cselect for <\/span>personalities that are afraid to fail.\u201d This selection process<span class=\"s1\"> combined with the learning environment puts students under immense pressure to succeed at any cost.<\/span><\/p>\n<p class=\"p1\"><span class=\"s1\">Many veterinary students have been taught to believe that being self-sufficient is an expectation in the profession.<sup>2<\/sup> There are unspoken assumptions that needing help is a sign of weakness, and \u201cif you fail out, it\u2019s because you weren\u2019t good enough to not fall apart in veterinary school,\u201d Dr. Hancock continues. In a recent qualitative study of barriers to help-seeking among veterinary students, approximately 61% of respondents felt that their problems were not important enough to warrant seeking help.<sup>2<\/sup> Even for those who want to seek help, barriers exist. In fact, lack of time was found to be the most common barrier to help-seeking among veterinary students.<sup>2<\/sup> This is often a function of strict attendance policies in classes and clinics, which Drs.\u00a0Hancock and Karaffa call presenteeism: \u201ca norm that places one\u2019s physical presence above all else.\u201d<sup>2<\/sup> This is further compounded by the large workload outside of school hours.<\/span><\/p>\n<p class=\"p1\"><span class=\"s1\">Ultimately, the culture of veterinary school is influenced by the profession at large. There are unwritten rules of success that elevate overwork, presenteeism, and perfection as ideals that should be adopted. Veterinary students often feel pressured to excel in school or risk their place as a member of the profession.<sup>2<\/sup> Some students fear that their mentors or role models will look poorly on them for not being able to get through school without needing support.<sup>2<\/sup> This narrative contributes further to a self-stigma that resists seeking help. Ultimately, the current culture of veterinary school and the unwritten expectations of the veterinary profession puts our young colleagues in a vulnerable position before they ever enter practice.<\/span><\/p>\n<h2 class=\"p2\">The Transition to Practice<\/h2>\n<p class=\"p1\"><span class=\"s1\">Early career veterinarians, a cohort that includes new and recent graduates, face many challenges with the transition from student to practicing veterinarian. These include financial pressures, imposter syndrome, transitioning to a leadership role, and moral and ethical dilemmas brought about by clients\u2019 inability or unwillingness to pursue the gold standard care often taught during veterinary school. While life in practice is very different from life in veterinary school in many ways, the cultural norms of the profession that were learned in veterinary school persist and are often reinforced in practice, such as the expectation of self-sufficiency. Students go from having \u201can army of people\u201d for case support (i.e., interns, residents, veterinary nurses, attending clinicians, student colleagues) to primary case management essentially overnight.<sup>3<\/sup> This can exacerbate fears of making mistakes and add an additional stressor.<\/span><\/p>\n<p class=\"p1\"><span class=\"s1\">Success during this transition period requires that young veterinarians have been trained not only in clinical skills and critical thinking, but also in communication and self-care skills.<sup>3<\/sup> The American Association of Veterinary Medical Colleges has recently included \u201cattends to well-being of self and others\u201d in their competencies-based framework for education.<sup>3<\/sup> While these non-clinical skills are being added to veterinary school curricula, change takes time, and there is a limit to how much information can be effectively taught in a 4-year program. Those who are <a href=\"https:\/\/todaysveterinarypractice.com\/personal-professional-development\/the-evolution-of-mentorship-for-veterinary-professionals\/\" target=\"_blank\" rel=\"noopener\">mentoring early career veterinarians<\/a> should be sure to discuss and model non-clinical skills for their mentees.<\/span><\/p>\n<h2 class=\"p2\">Shifting the Professional Culture<\/h2>\n<p class=\"p1\"><span class=\"s1\">Ongoing research into factors affecting mental health must continue. Dr. Hancock notes that \u201cmental health literature is always talking about individuals\u2014individual issues and individual solutions.\u201d While an individual focus is necessary, newer literature and proposed solutions should begin to focus on methods to create systemic change in the field.<\/span><\/p>\n<p class=\"p1\"><span class=\"s1\">One change in the veterinary profession that has been proposed by Galaxy Vets and supported by <a href=\"https:\/\/www.nomv.org\/\" target=\"_blank\" rel=\"noopener\">Not One More Vet<\/a> is to update the veterinary and veterinary technician oaths \u201cto include a commitment to attend to personal health and mental wellbeing.\u201d<sup>4,5<\/sup> The human medical field has already made a similar change, highlighting how important self-care is to maintain the highest level of care for patients. Dr. Zak knows that changing the oath won\u2019t fix the mental health challenges we face but feels it will be a positive step. Updating the language of the veterinary professional oaths offers a wide-reaching opportunity to show that the culture of veterinary medicine is more accepting of help-seeking and prioritizing self-care.<\/span><\/p>\n<p class=\"p1\"><span class=\"s1\">Cultural changes in the profession can\u2014and should\u2014happen at many levels. First, it is essential that students, faculty, and administrators in veterinary schools work to disrupt the current culture of perfectionism and presenteeism. One way this can be done, says Dr. Hancock, is to make it clear to students that it is OK to take time off to attend medical appointments, including therapy and mental health-related appointments. Openly discussing the mental health challenges that faculty have experienced and students are currently facing can also help to encourage help-seeking in those who need it.<\/span><\/p>\n<p class=\"p1\"><span class=\"s1\">Veterinary professionals outside of academia can play a role in shifting the culture as well, says Dr. Hancock, especially those who are mentoring pre-veterinary and veterinary students. Discussing our own experiences and times when we have struggled can help to normalize help-seeking and break the stigma of self-sufficiency. Additionally, Dr. Hancock encourages veterinarians to share and celebrate the many areas of our lives outside of the profession. This will help students see that veterinary medicine is only one facet of a veterinarian\u2019s life. Dr.\u00a0Zak exemplifies these recommendations. He has experienced significant burnout in his career and speaks openly of his mental health struggles. He feels that a change in professional expectations is needed and encourages veterinarians to \u201cstop promoting heroism in extended hours and sleep deprivation.\u201d <\/span><\/p>\n<p class=\"p1\"><span class=\"s1\">Through Galaxy Vets, Dr. Zak is working to change the way clinics are managed in order to promote mental health. He has recognized that many mental health struggles are \u201can outcome of poor management.\u201d His company relies on 3 pillars: supporting financial independence for employees, creating flexibility in the work schedule and type of work, and measuring outcomes to ensure the programs they implement are having the desired effect. He encourages clinics to create an \u201cenvironment of safety and trust,\u201d where employees can <\/span>identify when they are struggling and are comfortable in asking for help when they need it.<\/p>\n<p class=\"p1\"><span class=\"s1\">Dr. Zak\u2019s company is not alone in changing the way that practices run. As the culture of the profession shifts, more and more corporate groups and privately owned practices are implementing changes to support employees and provide flexibility. \u201cWe aren\u2019t doomed to mental illness as a profession,\u201d Dr. Hancock reminds veterinary professionals. Though it will take time, we can make positive change. \u201cCulture is not a given,\u201d she says. \u201cWe get to make it.\u201d <\/span><\/p>\n","protected":false},"excerpt":{"rendered":"<p>A
