{"id":22577,"date":"2020-12-15T15:35:41","date_gmt":"2020-12-15T15:35:41","guid":{"rendered":"https:\/\/todaysveterinarypractice.com\/?p=22577"},"modified":"2022-07-06T18:57:29","modified_gmt":"2022-07-06T18:57:29","slug":"treating-canine-superficial-pyoderma-in-the-era-of-antimicrobial-resistance","status":"publish","type":"post","link":"https:\/\/navc.sitepreview.app\/todaysveterinarypractice.com\/dermatology\/treating-canine-superficial-pyoderma-in-the-era-of-antimicrobial-resistance\/","title":{"rendered":"Treating Canine Superficial Pyoderma in the Era of Antimicrobial Resistance"},"content":{"rendered":"<p class=\"p1\"><span class=\"s1\">Canine superficial pyoderma, also called <i>bacterial folliculitis<\/i>, is one of the most common problems veterinarians face, and the increasing prevalence of staphylococcal antimicrobial resistance poses a new challenge to treatment. Failure to recognize staphylococcal antimicrobial resistance frequently results in ineffective empiric therapeutic choices and protracted clinical disease. In addition, concern exists regarding potential transmission of antimicrobial-resistant strains among animals and humans. <\/span><\/p>\n<p class=\"p1\"><span class=\"s1\">The rise of antimicrobial resistance requires veterinarians to shift their approach in treating bacterial skin infections, particularly in regard to the use and selection of systemic antimicrobials. Specific antimicrobial guidelines and clinical consensus on treating bacterial skin infections have been developed by the International Society for Companion Animal Infectious Diseases (ISCAID, <\/span><a href=\"http:\/\/iscaid.org\" target=\"_blank\" rel=\"noopener\"><span class=\"s2\">iscaid.org<\/span><\/a><span class=\"s1\">) and the World Association for Veterinary Dermatology (WAVD, <\/span><a href=\"http:\/\/wavd.org\" target=\"_blank\" rel=\"noopener\"><span class=\"s2\">wavd.org<\/span><\/a><span class=\"s1\">) to help veterinarians adequately diagnose and treat pyoderma, using judicious antimicrobial therapy to prevent bacterial resistance.<sup>1,2<\/sup> To treat canine pyoderma successfully, it is important to recognize its clinical signs, confirm the infection, and identify and treat any underlying cause to help prevent pyoderma recurrence. <\/span><\/p>\n<h2 class=\"p2\">Bacteria that Cause Superficial Pyoderma<\/h2>\n<p class=\"p1\"><span class=\"s1\"><i>Staphylococcus pseudintermedius<\/i>\u00a0is the most common cause of superficial pyoderma in dogs. Other less commonly identified gram-positive bacteria include <i>Staphylococcus schleiferi<\/i>, <i>Staphylococcus aureus, Staphylococcus xylosus, Staphylococcus epidermitis<\/i>, and <i>Streptococcus <\/i>species.<i> <\/i>Gram-negative bacteria such as <i>Pseudomonas aeruginosa <\/i>and<i> Corynebacterium <\/i>species are rarely identified.<sup>1<\/sup><\/span><\/p>\n<h2 class=\"p2\">Antimicrobial Resistance<\/h2>\n<p class=\"p1\"><span class=\"s1\">Antimicrobial resistance occurs when bacteria can survive in the presence of antimicrobials designed to kill them or stop their growth. One of the most important factors leading to the emergence of antimicrobial-resistant bacteria is the use and misuse of antimicrobial therapy. Therefore, reduction of the overall use of antimicrobials in veterinary medicine, especially systemic antimicrobials, can help reduce antimicrobial resistance. <\/span><\/p>\n<p class=\"p1\"><span class=\"s1\">Multidrug-resistant (MDR) bacteria are bacteria that demonstrate in vitro resistance to at least 3 antimicrobial classes. The term <i>methicillin-resistant staphylococcus<\/i> (MRS) refers to any <i>Staphylococcus <\/i>species that is resistant to oxacillin; that is, it demonstrates in vitro resistance to all <\/span><span class=\"s3\">\u03b2<\/span><span class=\"s1\">-lactam antimicrobials, including all cephalosporins, penicillins (including potentiated amoxicillin), and carbapenem antimicrobials. When an MRS expresses resistance to at least 2 additional antimicrobial classes, it is also referred to as MDR.<\/span><\/p>\n<h2 class=\"p2\">Clinical Signs<\/h2>\n<p class=\"p1\"><span class=\"s1\">Clinical signs of canine superficial pyoderma typically include erythematous papules and pustules (<\/span><span class=\"s2\"><b>FIGURE 1<\/b><\/span><span class=\"s1\">), which are usually associated with hair follicles. Pustules are quite short lived and not always identified during examination. Successively, scaling, crusts, and epidermal collarettes (<span class=\"s2\"><b>FIGURE<\/b><\/span><\/span><span class=\"s2\"><b>\u00a02<\/b><\/span><span class=\"s1\">) are often present. Multifocal areas of hypotrichotic or alopecic patches (moth-eaten alopecia; <span class=\"s2\"><b>FIGURE<\/b><\/span><\/span><span class=\"s2\"><b>\u00a03<\/b><\/span><span class=\"s1\">) can also be seen, most commonly in short-coated breeds. Moth-eaten alopecia on the dorsum of a dog.<\/p><\/div>\n<h2 class=\"p2\">Diagnosis<\/h2>\n<p class=\"p1\"><span class=\"s1\">The first step in treating superficial pyoderma properly is to confirm the diagnosis. The diagnosis includes recognition of the typical clinical signs, the exclusion of other similar conditions, and confirmation via skin cytology, plus or minus bacterial culture and susceptibility testing.<sup>3<\/sup> This approach helps prevent inappropriate use of antimicrobial therapy for skin diseases that are not associated with pyoderma. <\/span><\/p>\n<h3 class=\"p3\">Differential Diagnosis<\/h3>\n<p class=\"p1\"><span class=\"s1\">Other causes of folliculitis and pustular disease should be considered, including demodicosis, dermatophytosis, sterile pustular diseases such as pemphigus foliaceus (<span class=\"s2\"><b>FIGURE<\/b><\/span><\/span><span class=\"s2\"><b>\u00a04<\/b><\/span><span class=\"s1\">), and sterile neutrophilic or eosinophilic pustulosis. Generalized pemphigus foliaceus mimicking superficial pyoderma in a dog.<\/p><\/div>\n<h3 class=\"p3\">Skin Cytology<\/h3>\n<p class=\"p1\"><span class=\"s1\">Skin cytology is the most important diagnostic test for canine pyoderma; however, it is still underused. Before antimicrobials are administered, the diagnosis of superficial pyoderma needs to be supported by cytologic findings demonstrating coccoid bacteria, often associated with inflammatory cells (typically degenerate neutrophils) and, when present, bacteria phagocytosis (<span class=\"s2\"><b>FIGURE<\/b><\/span><\/span><span class=\"s2\"><b>\u00a05<\/b><\/span><span class=\"s1\">). Skin cytology from a dog with superficial pyoderma demonstrating degenerate neutrophils and intracellular (blue arrow) and extracellular (orange arrow) cocci bacteria.<\/p><\/div>\n<h3 class=\"p3\">Bacterial Culture and Susceptibility Testing<\/h3>\n<p class=\"p1\"><span class=\"s1\">Bacterial culture and susceptibility testing is a very important diagnostic tool for identification of the bacteria involved and selection of appropriate antimicrobial therapy (<\/span><span class=\"s2\"><b>BOX 1<\/b><\/span><span class=\"s1\">), especially when considering systemic antibiotics. The need for this test has increased with the emergence of MDR infections in veterinary medicine. Ideally, samples should be taken from pustules. If pustules are not present, samples may be collected from exudate beneath crusts, epidermal collarettes, and, if needed, from papules (by rupturing the papules with a 22-gauge needle). There is no current indication that use of antimicrobials affects isolation of causative bacteria in canine superficial pyoderma; therefore, samples may be collected for culture whenever indicated, regardless of administration of antimicrobials.<\/span><\/p>\n<div class=\"su-box su-box-style-soft\" id=\"\" style=\"border-color:#003d45;border-radius:3px;\"><div class=\"su-box-title\" style=\"background-color:#007078;color:#ffffff;border-top-left-radius:1px;border-top-right-radius:1px\">BOX 1 When to Perform Bacterial Culture and Sensitivity Testing<\/div><div class=\"su-box-content su-u-clearfix su-u-trim\" style=\"border-bottom-left-radius:1px;border-bottom-right-radius:1px\"><\/p>\n<ul>\n<li class=\"p1\"><span class=\"s1\">History of recurrent pyoderma <\/span><\/li>\n<li class=\"p1\"><span class=\"s1\">History of any antimicrobial use within previous 6 months <\/span><\/li>\n<li class=\"p1\"><span class=\"s1\">History of multiple courses of systemic antimicrobial therapy <\/span><\/li>\n<li class=\"p1\"><span class=\"s1\">History of poor response to previous or current systemic antimicrobial therapy <\/span><\/li>\n<li class=\"p1\"><span class=\"s1\">History of multidrug- or methicillin-resistant infections <\/span><\/li>\n<li class=\"p1\"><span class=\"s1\">Development of new skin lesions 2 weeks or more after initiation of systemic antimicrobial <\/span><\/li>\n<li class=\"p1\"><span class=\"s1\">Presence of residual skin lesions and cytologic evidence of bacteria after 6 weeks of systemic antimicrobial <\/span><\/li>\n<li class=\"p1\"><span class=\"s1\">Identification of intracellular rods on cytology without suspicion for contamination<\/span><\/li>\n<\/ul>\n<p><\/div><\/div>\n<h2 class=\"p2\">Therapy for Superficial Pyoderma<\/h2>\n<p class=\"p1\"><span class=\"s1\">There is no gold standard or \u201cone size fits all\u201d method of treatment.\u00a0The treatment must be tailored to each patient. The type of therapy selected should be based on the nature of the disease, the severity and extent of the lesions, concurrent diseases, any other administered drugs, the ability of the owner to administer topical and systemic therapy, and the owner\u2019s compliance and financial situation, in addition to any known or expected antimicrobial resistance. <\/span><\/p>\n<h3 class=\"p3\">Topical Antimicrobial Therapy<\/h3>\n<p class=\"p1\"><span class=\"s1\">Topical antimicrobial therapy can be of significant benefit in the treatment of canine superficial pyoderma, and its value has risen with emerging bacterial resistance (<\/span><span class=\"s2\"><b>BOX 2<\/b><\/span><span class=\"s1\">). Topical therapy is usually safer and reaches higher concentration in the skin compared with systemic antimicrobials. <i>To prevent the unnecessary use of systemic antimicrobials, topical therapy should always be recommended in cases of superficial pyoderma before systemic therapy is considered.<\/i> Topical therapy can be effective alone, particularly in cases of mild to moderate infection (<span class=\"s2\"><b>FIGURE<\/b><\/span><\/span><span class=\"s2\"><b>\u00a06<\/b><\/span><span class=\"s1\">), but it can also assist in more rapid resolution of lesions and reduce the duration of systemic antimicrobial use. The clinical consensus guidelines of the WAVD state that topical therapy using antibacterial agents with proven antistaphylococcal efficacy is the recommended treatment modality for any superficial pyoderma involving MDR or MRS, particularly those with localized lesions.<sup>2<\/sup><\/span><\/p>\n<div class=\"su-box su-box-style-soft\" id=\"\" style=\"border-color:#003d45;border-radius:3px;\"><div class=\"su-box-title\" style=\"background-color:#007078;color:#ffffff;border-top-left-radius:1px;border-top-right-radius:1px\">BOX 2 Benefits of Topical Therapy for Canine Superficial Pyoderma<\/div><div class=\"su-box-content su-u-clearfix su-u-trim\" style=\"border-bottom-left-radius:1px;border-bottom-right-radius:1px\"><\/p>\n<ul>\n<li class=\"p1\"><span class=\"s1\">Limited to the treated organ <\/span><\/li>\n<li class=\"p1\"><span class=\"s1\">High concentration on the skin <\/span><\/li>\n<li class=\"p1\"><span class=\"s1\">Fewer side effects <\/span><\/li>\n<li class=\"p1\"><span class=\"s1\">Faster resolution of the infection <\/span><\/li>\n<li class=\"p1\"><span class=\"s1\">Reduced systemic antimicrobial use or duration<\/span><\/li>\n<\/ul>\n<p><\/div><\/div>\n<p class=\"p1\"><span class=\"s1\">Shampoos, mousses, lotions, wipes, sprays, and rinses containing antiseptics such as chlorhexidine (commonly used by the author), benzoyl peroxide, ethyl lactate, hydroxyl acids (i.e., acetic acid, lactic acid, and malic acid), and hypochlorous acid may be used 1 to 3 times weekly up to daily for resolution of localized and generalized lesions, as well as for maintenance to help prevent recurrence of infection. A contact time of at least 5 to 10 minutes is important for optimal efficacy of antimicrobial shampoos. (B) Same dog with complete resolution of the pyoderma after topical therapy.<\/p><\/div>\n<p class=\"p1\"><span class=\"s1\">Ointments, creams, gels, and sprays containing gentamicin, bacitracin, neosporin, silver sulfadiazine, hydroxyl acids, or fusidic acid (not available in the United States) can be used 2 to 3 times daily in combination with baths or on non-bath days. These topical antibiotic therapies should be used until complete resolution of the skin infection. <\/span><\/p>\n<p class=\"p1\"><span class=\"s1\">Mupirocin, a carboxylic acid ointment, is highly effective against MRS skin infections and can be used for localized treatment; however, the author uses mupirocin only in resistant cases, when other topicals are not effective or demonstrate resistance in vitro,<i> <\/i>as this is the most important topical antimicrobial used to treat methicillin-resistant<i> S aureus <\/i>(MRSA) in humans. In the author\u2019s opinion, topical fluoroquinolones should not be used empirically and should also be saved <\/span>for more resistant cases if other topicals are not effective.<\/p>\n<p class=\"p1\"><span class=\"s1\">Skin infections associated with significant inflammation may benefit from short, limited courses of topicals containing glucocorticoids such as hydrocortisone, betamethasone, mometasone, or triamcinolone; however, care should be taken with these products, as chronic use of topical glucocorticoids can induce adverse cutaneous reactions (e.g., atrophic skin, wounds). <\/span><\/p>\n<p class=\"p1\"><span class=\"s1\">It is very important to educate pet owners about how to correctly apply topical therapy and maintain the correct schedule for the recommended duration.<\/span><\/p>\n<h3 class=\"p3\">Systemic Antimicrobial Therapy<\/h3>\n<p class=\"p1\"><span class=\"s1\">If selected properly, systemic antimicrobial therapy is very effective in cases of canine superficial pyoderma. The basic principles of successful systemic antimicrobial therapy include proper antimicrobial selection, establishment of an effective dosage, and long enough duration of treatment to ensure complete resolution of the infection. The choice of systemic antimicrobial may be empiric in first-time, nonrecurrent pyoderma cases and in dogs that have not been exposed to multiple courses of systemic antimicrobials. Otherwise, antimicrobial selection should always be based on results of culture and susceptibility testing. When a systemic antibiotic is needed, selection of one with the narrowest spectrum of activity possible will minimize the selection of resistant organisms.<sup>4<\/sup><\/span><\/p>\n<p class=\"p1\"><span class=\"s1\">The clinical consensus guidelines of the WAVD state that empirical drug selection for systemic therapy is always contraindicated when a multidrug-resistant infection is suspected based on historical factors, due to the high prevalence of multidrug resistance within these strains.<sup>2<\/sup> The ISCAID guidelines, last updated in 2014, outline 3 tiers of antimicrobials and their recommended use for canine superficial pyoderma.<sup>1<\/sup> Clinical use by veterinary dermatologists may vary from these guidelines. Guidelines for the diagnosis and antimicrobial therapy of canine superficial bacterial folliculitis (Antimicrobial Guidelines Working Group of the International Society for Companion Animal Infectious Diseases). <\/span><\/em><span class=\"s1\">Vet Dermatol<\/span><em><span class=\"s1\"> 2014;25(3):163-175.<\/span><\/em><\/p>\n<p><\/div><\/div>\n<p class=\"p1\"><span class=\"s1\"><b>First-tier antimicrobials<\/b> are those that can be used empirically (without culture and susceptibility testing in dogs with no or rare previous antimicrobial exposure) for cases of superficial pyoderma. Many dermatologists first select cephalexin or cefadroxil for empiric treatment of superficial pyoderma, with the second choice typically being amoxicillin clavulanate. Potentiated sulfonamides are often efficacious against <i>S pseudintermedius<\/i>; however, they can have many side effects (e.g., hypothyroidism, keratoconjunctivitis sicca, hypersensitivity reaction in certain breeds), especially when given for a long period of time. <\/span><\/p>\n<p class=\"p1\"><span class=\"s1\"><b>Second-tier antimicrobials<\/b> should only be used when culture and sensitivity results indicate that the bacterial isolate is not susceptible to any of the first-tier antimicrobials or when the patient experiences side effects from first-tier antimicrobials. Second-tier antimicrobials should always be used based on adequate in vitro susceptibility testing. The author usually considers clindamycin or doxycycline as the first choices for second-tier antimicrobials for MDR or MRS cases, based on their efficacy for these infections and their safety profile. <\/span><\/p>\n<p class=\"p1\"><span class=\"s1\">The ISCAID guidelines include clindamycin and lincomycin in the first tier of antimicrobials.<sup>1<\/sup> The author prefers to reserve these agents for second-tier use based on culture and susceptibility testing when other choices, such as cephalosporins, are not options. Several published reports, including one in dogs,<sup>6<\/sup> recommend the use of clindamycin only based on culture and susceptibility results due to a phenomenon called MLSB\u00a0(macrolide-lincosamide-streptogramin B) that is associated with cross-inducible resistance among macrolides. Therefore, when considering the use of clindamycin, it is important to verify that all tested macrolides show in vitro susceptibility. <\/span><\/p>\n<p class=\"p1\"><span class=\"s1\">Chloramphenicol, rifampin, and aminoglycosides are often last-resort options for MDR staphylococcal infections, mostly due to more severe potential adverse effects. Chloramphenicol and rifampin can be used safely in most dogs; however, close monitoring for potential aplastic anemia and liver toxicity, respectively, is needed. Historically, rifampin was used in combination with other antimicrobials (e.g.,\u00a0doxycycline) due to concern with development of fast resistance; however, polyantimicrobial systemic therapy should be avoided due to potential for more resistance. Additionally, a recent study showed that rifampin, used as monotherapy, was shown to be efficacious in almost 72% of dogs with MDR methicillin-resistant <i>Staphylococcus<\/i> pyoderma;<sup>8<\/sup> however, patients should be carefully monitored for resistance during treatment, especially when several months of treatment are needed. <\/span><\/p>\n<p class=\"p1\"><span class=\"s1\">The author considers cefovecin and cefpodoxime to be second-tier antimicrobials because they are third-generation cephalosporins, with a broader spectrum than other cephalosporins such as cephalexin and cefadroxil.<sup>7<\/sup> Additionally, extended-spectrum <\/span><span class=\"s3\">\u03b2<\/span><span class=\"s1\">-lactamase (ESBL) <i>Escherichia coli<\/i> has been reported in dogs, and third-generation cephalosporins are considered risk factors for infections associated with MRSA and ESBL <i>E coli<\/i> in humans. It is important to remember that none of the cephalosporins should be used for the treatment of MRS infections, which are resistant to these antimicrobials. <\/span><\/p>\n<p class=\"p1\"><span class=\"s1\"><b>Third-tier antimicrobials<\/b> (fluoroquinolones) should only be used when in vitro susceptibility is demonstrated and first- and second-tier antimicrobials are not effective.<sup>7<\/sup> The author considers these antimicrobials as last options for treating canine pyoderma, unless they need to be selected for safety reasons for the patient. Ciprofloxacin should not be used because of its reported poor absorption and bioavailability in dogs.<\/span><\/p>\n<p class=\"p1\"><span class=\"s1\"><b>Fourth-tier antimicrobials<\/b> should not be used in veterinary medicine, particularly with regard to routine cases of superficial pyoderma. They should be reserved <\/span>for the treatment of severe MRSA infections in humans.<\/p>\n<h3 class=\"p3\">General Guidelines for Systemic Antimicrobial Use<\/h3>\n<p class=\"p1\"><span class=\"s1\">Using the correct dose, frequency of administration, and duration of antimicrobials is crucial for a successful outcome when treating any infection. Antimicrobials given more frequently than once a day (e.g., cephalexin, clindamycin) are called <i>time dependent<\/i>. To be effective, their concentration at the infection site must be above the minimal inhibitory concentration (MIC) of the bacteria, ideally for the duration of the administration interval, but at least 75% of that time is acceptable. Time-dependent antimicrobials should not be given less frequently than the recommended interval, and increasing their dose does not necessarily increase efficacy. <\/span><\/p>\n<p class=\"p1\"><span class=\"s1\">Antimicrobials given only once daily (e.g., enrofloxacin, marbofloxacin) are called <i>dose dependent. <\/i>The <\/span>important consideration when using these antimicrobials<span class=\"s1\"> is how much their concentration at the infection site is above the bacterial MIC. For these antimicrobials, increasing the dose increases efficacy but increasing the frequency of administration does not. <\/span><\/p>\n<p class=\"p1\"><span class=\"s1\">Subtherapeutic doses of systemic antimicrobials, including pulse therapy protocols, are currently strongly discouraged as they are likely to promote antimicrobial resistance. <\/span><\/p>\n<p class=\"p1\"><span class=\"s1\">Recommended doses of systemic drugs for canine superficial pyoderma have been published in the ISCAID guidelines<sup>1<\/sup> and elsewhere.<\/span><\/p>\n<h2 class=\"p2\">Treatment Duration, Prevention, And Control Of Canine Superficial Pyoderma<\/h2>\n<p class=\"p1\"><span class=\"s1\">The typical duration of treatment for superficial pyoderma is 3 to 4 weeks, but more time may be needed. It is currently recommended by veterinary dermatologists to treat superficial pyoderma until 1 week after complete clinical resolution of the infection. Patients with pyoderma should always be reevaluated by the veterinarian, who should not rely solely on the owner\u2019s perception to determine if the bacterial infection has resolved. To prevent frequent recurrence of superficial pyoderma, it is crucial to try to identify and control the underlying primary disease (e.g., allergy, endocrinopathy) that typically predisposes dogs to pyoderma. Educating clients about their pet\u2019s pyoderma and treatment has been shown to improve compliance. Additionally, owners should be educated about the need to avoid or minimize systemic antimicrobial use so they can be part of the solution to the global fight against antimicrobial resistance.
