{"id":20955,"date":"2020-05-06T19:18:35","date_gmt":"2020-05-06T19:18:35","guid":{"rendered":"https:\/\/todaysveterinarypractice.com\/?p=20955"},"modified":"2022-02-18T20:50:49","modified_gmt":"2022-02-18T20:50:49","slug":"cornell-canine-heart-surgery","status":"publish","type":"post","link":"https:\/\/navc.sitepreview.app\/todaysveterinarypractice.com\/news\/cornell-canine-heart-surgery\/","title":{"rendered":"Cornell Veterinarians and Physicians Partner for Rare Canine Open-Heart Surgery"},"content":{"rendered":"<p>ITHACA, N.Y. \u2013 Imagine the time it takes to brew some coffee, answer an email or take out the trash \u2014 approximately five minutes, maybe less. In that amount of time, Cornell veterinarians and human medicine physicians opened a canine patient\u2019s heart to remove a tumor blocking her right ventricle.<\/p>\n<p>The dog, a seven-year-old yellow Labrador retriever named Lucy, started fainting regularly late last year. Her owners, the Baginski family, were concerned by this unusual behavior. \u201cThere were a few times when she became disoriented, and others when she\u2019d get excited while playing but then fall to the floor,\u201d said David Baginski.<\/p>\n<p>In their search for answers, the Baginskis were referred to the Cornell University College of Veterinary Medicine (CVM). There, the cardiologists carried out a 3D echocardiogram that showcased the tumor.<\/p>\n<p>\u201cLucy was periodically collapsing and essentially blacking out because of her heart\u2019s failure to oxygenate,\u201d said Dr. Galina Hayes, assistant professor of small animal surgery. \u201cThe mass was blocking the outflow of blood. If you imagine the ventricle is like a pump, it would be as if something got caught in the hose.\u201d<\/p>\n<p>The veterinarians were concerned that even if the cancer itself wasn\u2019t terminal, the effects of the tumor could be. The Baginskis agreed, worried that one day Lucy wouldn\u2019t wake up from a faint. \u201cAt first we thought she was just out of shape,\u201d Baginski said. \u201cWe\u2019d have to encourage her to keep up on walks, but now we know it was a circulation problem and she was getting tired.\u201d<\/p>\n<div class=\"su-youtube su-u-responsive-media-yes\"><iframe width=\"600\" height=\"400\" src=\"https:\/\/www.youtube.com\/embed\/U-Q51uR9_nI?\" frameborder=\"0\" allowfullscreen allow=\"autoplay; encrypted-media; picture-in-picture\" title=\"Cornell veterinarians, physicians team up to tackle rare canine open-heart surgery\"><\/iframe><\/div>\n<p>Hayes conferred with colleagues at the Cornell University Hospital for Animals on the best approach to remove the mass. \u201cI\u2019ve never seen a case like this in my career,\u201d Hayes said. \u201cThere is maybe a total of three such reported cases in veterinary literature.\u201d<\/p>\n<p>One of the experts Hayes brought in was Dr. Luis Campoy, associate clinical professor and section chief of anesthesiology and pain medicine.<\/p>\n<p>\u201cI was on the phone with Dr. Hayes, and we knew we had to do something quickly, otherwise Lucy might not survive,\u201d said Campoy. \u201cIf we waited too long to decide what to do she could be gone.\u201d<\/p>\n<h2>Joining human and animal medicine<\/h2>\n<p>Campoy reached out to his counterparts at Weill Cornell Medicine&#8217;s Department of Anesthesiology in New York City to brainstorm options. Partnering with Weill Cornell Medicine is not a new concept for the Ithaca veterinary anesthesia team. For the last five years, CVM and Weill Cornell Medicine have engaged in an elective rotation for their respective anesthesiology residents. Dr. Maria Walline, a cardiothoracic anesthesiology fellow with Weill Cornell Medicine, jumped on an early morning shuttle bus from New York City to assist with Lucy\u2019s anesthesia.<\/p>\n<p>\u201cWe knew this would be a high-risk procedure, riskier than it would be with a human patient because it was not possible to use cardiopulmonary bypass for Lucy,\u201d said Walline, who completed a fellowship in pediatric anesthesia and is currently completing another in adult cardiothoracic anesthesia, two fields that align surprisingly well with veterinary medicine and this case in particular. \u201cThe patient profiles are similar in pediatric and veterinary anesthesia \u2014 you are treating a patient, typically smaller in size than an adult, who is non-verbal.\u201d<\/p>\n<p>Open-heart surgeries are more common in human patients, largely due to human hospitals\u2019 access to cardiopulmonary bypass machines, which can redirect blood flow for an extended period of time. Most animal hospitals lack this technology, Cornell\u2019s included, so Lucy\u2019s care team was left with one option: open-heart surgery without the safety net of a cardiopulmonary bypass machine.<\/p>\n<h2>Racing the clock<\/h2>\n<p>The surgeons would have to make the procedure exceptionally short, since the amount of time a body can survive and recover when the heart is not functioning is limited. \u201cAt maximum, it\u2019s about four minutes,\u201d Hayes said.<\/p>\n<p>\u201cWe were told that it was about a 50\/50 chance of Lucy surviving,\u201d Baginski said.<\/p>\n<div id=\"attachment_20959\" style=\"width: 210px\" class=\"wp-caption alignright\"><img fetchpriority=\"high\" decoding=\"async\" aria-describedby=\"caption-attachment-20959\" class=\"wp-image-20959 size-medium\" src=\"https:\/\/todaysveterinarypractice.com\/wp-content\/uploads\/sites\/4\/2020\/05\/lucy-labrador-retreiver-cornell-200x300.jpg\" alt=\"lucy labrador retriever cornell surgery\" width=\"200\" height=\"300\" srcset=\"https:\/\/navc.sitepreview.app\/todaysveterinarypractice.com\/wp-content\/uploads\/sites\/4\/2020\/05\/lucy-labrador-retreiver-cornell-200x300.jpg 200w, https:\/\/navc.sitepreview.app\/todaysveterinarypractice.com\/wp-content\/uploads\/sites\/4\/2020\/05\/lucy-labrador-retreiver-cornell.jpg 667w\" sizes=\"(max-width: 200px) 100vw, 200px\" \/><p id=\"caption-attachment-20959\" class=\"wp-caption-text\">Photo: Cornell Vet<\/p><\/div>\n<p>The surgical team had a four-minute window to stop the blood flow into the heart, make the incision to remove the mass, close the ventricular wall and release the blood so it could flow again. It took an array of experts to make sure that everything was timed impeccably.<\/p>\n<p>\u201cIt had to happen like clockwork,\u201d Campoy said. \u201cTo make a procedure like this a success, everyone has to execute their roles perfectly at the perfect time, even more than usual.\u201d<\/p>\n<p>\u201cSomeone called a countdown every 30 seconds once we occluded the vessels,\u201d said Sara Cantini, a fourth-year veterinary student who observed the surgery. \u201cIf the procedure lasts longer than four minutes, she could have permanent damage due to prolonged decrease in oxygen supply.\u201d<\/p>\n<p>In four minutes, surgeons removed approximately 90 percent of the mass threatening Lucy\u2019s heart. Though the most critical moments of the procedure took place during those four minutes, the complete procedure lasted approximately five hours, further highlighting the preparation and coordination necessary to execute the most crucial step. She spent the first night after surgery in the intensive care unit, and three days later, once she was deemed stable, Lucy was able to go home.<\/p>\n<h2>On the road to recovery<\/h2>\n<p>After her operation, Lucy stopped fainting every day and quickly returned to her former self. \u201cWhen we go out for walks today, she never tires \u2014 she\u2019s back to the way she was two or even three years ago,\u201d Baginski said.<\/p>\n<p>\u201cShe\u2019s done fantastic in the recovery process and we\u2019re really pleased with the long way she\u2019s come,\u201d Cantini said. \u201cShe\u2019s a tough girl.\u201d<\/p>\n<p>Campoy noted how multiple services, including soft tissue surgery, anesthesia, cardiology and critical care helped Lucy pull through: \u201cEveryone mobilized and accomplished this on top of their everyday duties.\u201d<\/p>\n<p>Cornell tested the mass and determined Lucy had ectopic thyroid carcinoma. In this case she is lucky: A targeted therapy that has few side effects and minimal risk can remove the remaining 10 percent of the tumor.<\/p>\n<div id=\"attachment_20958\" style=\"width: 760px\" class=\"wp-caption aligncenter\"><img decoding=\"async\" aria-describedby=\"caption-attachment-20958\" class=\"wp-image-20958 \" src=\"https:\/\/todaysveterinarypractice.com\/wp-content\/uploads\/sites\/4\/2020\/05\/lucy-labrador-retriever-cornell-veterinary.jpg\" alt=\"lucy labrador retriever cornell surgery\" width=\"750\" height=\"422\" srcset=\"https:\/\/navc.sitepreview.app\/todaysveterinarypractice.com\/wp-content\/uploads\/sites\/4\/2020\/05\/lucy-labrador-retriever-cornell-veterinary.jpg 1000w, https:\/\/navc.sitepreview.app\/todaysveterinarypractice.com\/wp-content\/uploads\/sites\/4\/2020\/05\/lucy-labrador-retriever-cornell-veterinary-300x169.jpg 300w, https:\/\/navc.sitepreview.app\/todaysveterinarypractice.com\/wp-content\/uploads\/sites\/4\/2020\/05\/lucy-labrador-retriever-cornell-veterinary-768x432.jpg 768w\" sizes=\"(max-width: 750px) 100vw, 750px\" \/><p id=\"caption-attachment-20958\" class=\"wp-caption-text\">Photo: Cornell Vet<\/p><\/div>\n<p>There are few animal hospitals in the country that can perform this procedure, and if Cornell had the equipment to perform cardiac bypass, more cases could be safely treated and with greater control, said Hayes.<\/p>\n<p>While Lucy may now have a lengthy scar down her chest, the Baginskis are glad she has regained her quality of life \u2014 returning to her favorite pastimes of swimming and greeting the neighbors. In March, before the COVID-19 pandemic hit the U.S. in full force, they were able to travel to the University of Missouri for the specific treatment Lucy needs to fight ectopic thyroid carcinoma. She\u2019s now making a full recovery and has had no further fainting episodes.<\/p>\n<p>\u201cI can\u2019t overemphasize how wonderfully we were treated at Cornell,\u201d Baginski said. \u201cThey helped us through the tough decisions \u2014 the doctors and staff and students, everyone.\u201d<\/p>\n<p><strong>Note:<\/strong> This article was <a
