A plethora of website resources are also available.<\/p>\n<hr \/>\n<p class=\"p1\"><span class=\"s1\">Providing medical care for puppies and kittens is one of the perks of our profession and, for many of us, offsets the daily routine of a busy veterinary practice.<\/span><\/p>\n<p class=\"p1\"><span class=\"s1\">With the joy of handling such little creatures, however, comes great responsibility. It is the veterinary team&#8217;s job, in general practice, to maintain a life-long, high-quality human\u2013animal bond. The first 4 months of life are a critical time period with regard to achieving that goal.<\/span><\/p>\n<h2 class=\"p3\"><span class=\"s1\"><b>CURRENT TRENDS IN WELLNESS CARE<\/b><\/span><\/h2>\n<p class=\"p1\"><span class=\"s1\">The 2011 Bayer Veterinary Care Usage Study identified several alarming trends, two of which identified that:<\/span><span class=\"s2\"><sup>1,2<\/sup><\/span><\/p>\n<ul class=\"ul1\">\n<li class=\"li1\"><span class=\"s1\">Veterinary visits have been declining since 2001<\/span><\/li>\n<li class=\"li1\"><span class=\"s1\">Pet owners no longer believe in the value of preventive care.<\/span><\/li>\n<\/ul>\n<p>Three facts from the study are pertinent to pediatric wellness care:<\/p>\n<ol>\n<li><b>Pet owners are confused about where to go for veterinary care.<\/b> What used to be the only choice\u2014the local, traditional veterinary office\u2014is just one of many options, which include:\n<ul>\n<li><span class=\"s1\">Vaccine clinics offered by pet &#8220;super stores&#8221;<\/span><\/li>\n<li><span class=\"s1\">Nonprofit clinics associated with the local SPCA or animal control <\/span><\/li>\n<li><span class=\"s1\">Expansive multidoctor specialty hospitals.<\/span><\/li>\n<\/ul>\n<\/li>\n<li><b><\/b><span class=\"s1\"><b>Pet owners are not aware of the value of an annual wellness examination.<\/b> Vaccination protocols have changed, allowing more than 1 year between some vaccinations, but veterinary teams, as a whole, have not made it clear to clients that wellness care encompasses much more than just vaccination.<\/span><\/li>\n<li><b><\/b><span class=\"s1\"><b>Pet owners prefer to see the same veterinarian as much as possible.<\/b> The new catchword for this preference is <i>relationship-centered care<\/i>. This concept has been around for decades, but its importance in veterinary care has just recently come to the forefront.<\/span><\/li>\n<\/ol>\n<p class=\"p1\"><span class=\"s1\">See <b>The Bayer Veterinary Care Usage Study<\/b> for further information on study findings.<\/span><\/p>\n<div class=\"orange-box\">\n<h2 class=\"p3\"><span class=\"s1\"><b>THE FIRST VISIT BEGINS ON THE PHONE<\/b><\/span><\/h2>\n<p class=\"p1\"><span class=\"s1\"><em>Relationship building<\/em> begins with the first phone call a client makes to inquire about setting up an appointment for his or her new puppy or kitten. Despite the hectic routine of a busy practice, whoever speaks with this client needs to take a moment to communicate genuine interest and willingness to help this person or family get the pet started off on the right &#8220;paw.&#8221; <\/span><\/p>\n<p class=\"p1\"><span class=\"s1\">This first phone call provides an opportunity to gather information, such as:<\/span><\/p>\n<ul class=\"ul1\">\n<li class=\"li1\"><span class=\"s1\">Does the owner\/family have previous experience with this species, or do they need some information, even before the initial pediatric visit? <\/span><\/li>\n<li class=\"li1\"><span class=\"s1\">Is the owner facing any challenges, such as house-training or feeding? Assure them that these issues can be discussed during the first visit, but also offer additional resources, for example: <\/span>\n<ul class=\"ul2\">\n<li class=\"li1\"><span class=\"s1\">The practice website, where, ideally, an educational section exists <strong>or <\/strong><\/span><\/li>\n<li class=\"li1\"><span class=\"s1\">A list of qualified pet trainers that work in the area.<\/span><\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<p class=\"p1\"><span class=\"s1\">Finally, &#8220;sway&#8221; new clients to your practice by highlighting any pertinent &#8220;perks&#8221; your practice offers:<\/span><\/p>\n<ul class=\"ul1\">\n<li class=\"li1\"><span class=\"s1\">Do you offer comprehensive or discounted pediatric wellness plans? <\/span><\/li>\n<li class=\"li1\"><span class=\"s1\">How about puppy or kitten socialization classes? <\/span><\/li>\n<li class=\"li1\"><span class=\"s1\">Does a groomer visit and provide mini-clips of the face and feet?<\/span><\/li>\n<\/ul>\n<p class=\"p1\"><span class=\"s1\">And don&#8217;t forget to ask clients to bring a stool sample with them to that first visit\u2014it saves patients from &#8220;the loop&#8221; and clients from repeated car trips.<\/span><\/p>\n<\/div>\n<h2 class=\"p3\"><span class=\"s1\"><b>WELLNESS CARE GOALS &amp; EDUCATION<\/b><\/span><\/h2>\n<h3 class=\"p4\"><span class=\"s1\"><b>Pediatric Wellness Care Goals<\/b><\/span><\/h3>\n<p class=\"p1\"><span class=\"s1\">Based on the study results, pediatric wellness care goals are changing. Protecting puppies and kittens from infectious diseases and preventing\/eliminating intestinal parasites are still crucial, but the new goals are much broader:<\/span><\/p>\n<ul>\n<li><span class=\"s1\">Develop a genuine relationship between the practice and pet owner. The components and benefits of relationship-centered care are discussed elsewhere<\/span><span class=\"s2\"><sup>3,4<\/sup><\/span><span class=\"s1\"> but focus on:<\/span>\n<ul>\n<li><span class=\"s1\">Partnering with clients to make health care decisions <\/span><\/li>\n<li><span class=\"s1\">Improving communication with clients to develop trust, which leads to enhanced adherence to veterinary recommendations.<\/span><\/li>\n<\/ul>\n<\/li>\n<li><span class=\"s1\">Communicate the value of preventive health care. In the &#8220;crunch&#8221; of daily practice, though, some pieces may be skipped. Therefore, it is critical that the veterinary team understands the importance of developing relationships with clients and communicating why wellness\/preventive care is imperative.<\/span><\/p>\n<p class=\"p1\"><span class=\"s1\">For example, client awareness can be enhanced by explaining that:<\/span><\/p>\n<ul class=\"ul1\">\n<li class=\"li1\"><span class=\"s1\">Heartworm disease is prevalent across the United States<\/span><\/li>\n<li class=\"li1\"><span class=\"s1\">Immunocompromised adults and children are vulnerable to parasites commonly encountered in pediatric pets<\/span><\/li>\n<li class=\"li1\"><span class=\"s1\">Pets are relinquished daily due to behavioral problems that began at a young age but were not addressed.<\/span><\/li>\n<\/ul>\n<p><a href=\"https:\/\/todaysveterinarypractice.com\/wp-content\/uploads\/sites\/4\/2013\/01\/010213-tt-ped-care.jpg\"><img fetchpriority=\"high\" decoding=\"async\" class=\"size-large wp-image-9605\" src=\"https:\/\/todaysveterinarypractice.com\/wp-content\/uploads\/sites\/4\/2013\/01\/010213-tt-ped-care-537x1024.jpg\" alt=\"EpubXpress - Text PDF\" width=\"537\" height=\"1024\" srcset=\"https:\/\/navc.sitepreview.app\/todaysveterinarypractice.com\/wp-content\/uploads\/sites\/4\/2013\/01\/010213-tt-ped-care-537x1024.jpg 537w, https:\/\/navc.sitepreview.app\/todaysveterinarypractice.com\/wp-content\/uploads\/sites\/4\/2013\/01\/010213-tt-ped-care-157x300.jpg 157w, https:\/\/navc.sitepreview.app\/todaysveterinarypractice.com\/wp-content\/uploads\/sites\/4\/2013\/01\/010213-tt-ped-care-768x1464.jpg 768w, https:\/\/navc.sitepreview.app\/todaysveterinarypractice.com\/wp-content\/uploads\/sites\/4\/2013\/01\/010213-tt-ped-care-806x1536.jpg 806w, https:\/\/navc.sitepreview.app\/todaysveterinarypractice.com\/wp-content\/uploads\/sites\/4\/2013\/01\/010213-tt-ped-care-1075x2048.jpg 1075w, https:\/\/navc.sitepreview.app\/todaysveterinarypractice.com\/wp-content\/uploads\/sites\/4\/2013\/01\/010213-tt-ped-care-scaled.jpg 1343w\" sizes=\"(max-width: 537px) 100vw, 537px\" \/><\/a><\/p>\n<h2>VACCINATION PROTOCOLS<\/h2>\n<h3 class=\"p4\"><span class=\"s1\"><b>Vaccine Recommendations<\/b><\/span><\/h3>\n<p class=\"p1\"><span class=\"s1\">Most veterinary schools now embrace, based on evidence, the concept of dividing vaccines into: <\/span><\/p>\n<ul class=\"ul1\">\n<li class=\"li1\"><b><\/b><span class=\"s1\"><b>Core:<\/b> These vaccines should be administered to all dogs and cats. <\/span><\/li>\n<li class=\"li1\"><b><\/b><span class=\"s1\"><b>Noncore:<\/b> These vaccines are only administered based on risk of exposure to the pathogen.<\/span><\/li>\n<\/ul>\n<h3 class=\"p4\"><span class=\"s1\"><b>Initial Vaccination Series<\/b><\/span><\/h3>\n<p class=\"p1\"><span class=\"s1\">For core vaccines, an initial series is required to develop adequate immunity. <\/span><\/p>\n<ul class=\"ul1\">\n<li class=\"li1\"><span class=\"s1\">An initial vaccine series is usally defined as 2 to 3 doses administered between 6 and 16 weeks of age (8, 12, and 16 weeks are commonly recommended).<\/span><span class=\"s2\"><sup>5<\/sup><\/span><\/li>\n<li class=\"li1\"><span class=\"s1\">Doses should be administered <em>no less than 2<\/em> weeks apart and <em>no more than 6<\/em> weeks apart. <\/span><\/li>\n<li class=\"li1\"><span class=\"s1\">Vaccination creates temporary immunosuppression; revaccinating at <em>less than<\/em> a 2-week interval prevents a complete response to the next dose of vaccine. <\/span><\/li>\n<li class=\"li1\"><span class=\"s1\">Additional vaccine doses administered <em>more than<\/em> 6 weeks after the initial dose are not expected to trigger the development of a memory response on subsequent exposure to the pathogen.<\/span><span class=\"s2\"><sup>6<\/sup><\/span><\/li>\n<\/ul>\n<h3 class=\"p4\"><span class=\"s1\"><b>Maternal Antibodies<\/b><\/span><\/h3>\n<p class=\"p1\"><span class=\"s1\">Puppies and kittens receive antibodies for infectious diseases through colostrum consumed in the first 24 hours of nursing. However, significant amounts of maternal antibodies can interfere with a pediatric patient&#8217;s ability to respond to vaccination.<\/span><\/p>\n<p class=\"p1\"><span class=\"s1\">\u2022 <strong>Puppies<\/strong> should receive the final dose of a core vaccine series between 14 and 16 weeks because, even at 12 weeks of age, maternal antibody levels may still be sufficiently high to interfere with vaccination.<\/span><span class=\"s2\"><sup>5<\/sup><\/span><\/p>\n<p class=\"p1\"><span class=\"s1\">\u2022 <strong>Kittens<\/strong> should receive the final dose of a core vaccine series at 16 weeks of age to ensure protective immunity.<\/span><span class=\"s2\"><sup>7<\/sup><\/span><\/p>\n<p class=\"p1\"><span class=\"s1\">A full discussion on vaccination strategies and adverse events is beyond the scope of this article. Articles are available at todaysveterinarypractice.com.<\/span><\/p>\n<p align=\"LEFT\">Two tables detailing pediatric vaccination protocols\u2014<a href=\"https:\/\/todaysveterinarypractice.com\/clinical-resources\/\" target=\"_blank\" rel=\"noopener noreferrer\"><strong>Vaccination <\/strong><strong>Recommendations for Puppies<\/strong> <\/a>and <a href=\"https:\/\/todaysveterinarypractice.com\/clinical-resources\/\" target=\"_blank\" rel=\"noopener noreferrer\"><strong>Vaccination Recommendations for Kittens<\/strong><\/a>\u2014are available at todaysveterinarypractice.com (Resources).<\/p>\n<h2 class=\"p3\"><span class=\"s1\"><b>PARASITE MANAGEMENT<\/b><\/span><\/h2>\n<p class=\"p1\"><span class=\"s1\">In my experience, there are 4 components of pediatric parasite management:<\/span><\/p>\n<ol class=\"ol1\">\n<li class=\"li1\"><span class=\"s1\"><strong>Empiric deworming<\/strong> for hookworms and roundworms<\/span><\/li>\n<li class=\"li1\"><span class=\"s1\"><strong>Heartworm prophylaxis<\/strong> initiated <\/span><\/li>\n<li class=\"li1\"><span class=\"s1\"><strong>Ectoparasite prevention<\/strong> evaluated<\/span><\/li>\n<li class=\"li1\"><span class=\"s1\"><strong>Fecal flotation<\/strong> by centrifugation<\/span><\/li>\n<\/ol>\n<div class=\"orange-box\">\n<p align=\"LEFT\">Visit todaysveterinarypractice.com to read and download <a href=\"https:\/\/todaysveterinarypractice.com\/clinical-resources\/\" target=\"_blank\" rel=\"noopener noreferrer\"><strong>Parasites &amp; Zoonotic Disease: Client Education<\/strong><\/a>, available in the resources section.<\/p>\n<\/div>\n<h3><span class=\"s1\"><b>Empiric Deworming<\/b><\/span><\/h3>\n<p class=\"p1\"><span class=\"s1\">There are 2 reasons that parasitic infestations may be present in pediatric patients (as young as 2 weeks of age):<\/span><\/p>\n<ol class=\"ol1\">\n<li class=\"li1\"><span class=\"s1\">The high prevalence of hookworms\/roundworms in canine and feline populations, whether in the environment or due to maternal transmission.<\/span><\/li>\n<li class=\"li1\"><span class=\"s1\">A parasite life cycle that involves transcolostral or transplacental transmission of larvae. <\/span><\/li>\n<\/ol>\n<p class=\"p1\"><span class=\"s1\">The patient should be dewormed during the first pediatric visit, with either: <\/span><\/p>\n<ul class=\"ul1\">\n<li class=\"li1\"><span class=\"s1\">Combination products effective against other parasites (eg, Heartgard Plus, merial.com; Trifexis, elanco.com; Advantage Multi, bayer-ah.com)<strong> or<\/strong><\/span><\/li>\n<li class=\"li1\"><span class=\"s1\">If age and\/or weight require it, pyrantel pamoate.<\/span><\/li>\n<li class=\"li1\"><span class=\"s1\">Deworming should be repeated at each pediatric wellness visit.<\/span><\/li>\n<\/ul>\n<p class=\"p1\"><span class=\"s1\">Deworming a puppy or kitten provides the opportunity to discuss these parasites&#8217; zoonotic potential, especially if young children are in the home. Veterinary practices are increasingly being held accountable for damages when children are infected with visceral larva migrans after exposure to new puppies.<\/span><span class=\"s2\"><sup>8<\/sup><\/span><\/p>\n<h3 class=\"p4\"><span class=\"s1\"><b>Heartworm Prophylaxis<\/b><\/span><\/h3>\n<p class=\"p1\"><span class=\"s1\">The 2011 American Veterinary Medical Association\/American Animal Hospital Association <strong>Canine and Feline Preventive Health Care Guidelines<\/strong> recommend that all cats and dogs in the United States receive year-round heartworm preventives, regardless of heartworm prevalence in a community.<\/span><span class=\"s2\"><sup>9<\/sup><\/span><\/p>\n<p class=\"p1\"><span class=\"s1\">The pediatric examination allows the veterinary team to evaluate clients&#8217; knowledge of heartworm disease as well as explain the difference between heartworm disease in dogs and cats. The zoonotic disease discussion previously mentioned can be segued into a conversation about the necessity of heartworm prevention.<\/span><\/p>\n<p class=\"p1\"><span class=\"s1\">Administration of heartworm prophylaxis differs among the type of preventive (ie, monthly topical versus 6-month injection). The key point is that the importance of prevention should be reiterated at each pediatric visit.<\/span><\/p>\n<h3 class=\"p4\"><span class=\"s1\"><b>Ectoparasite Prevention<\/b><\/span><\/h3>\n<p class=\"p1\"><span class=\"s1\">The importance of flea and tick prevention in puppies and kittens will depend on where in the U.S. the veterinary practice is located, as ectoparasite prevalence varies greatly by geographic region. If location, history, or examination indicates a need for flea\/tick prevention prior to 16 weeks of age, the veterinarian can recommend:<\/span><\/p>\n<ul class=\"ul1\">\n<li class=\"li1\"><span class=\"s1\">Use of a <strong>single, &#8220;combination&#8221; product<\/strong> that deworms, prevents heartworm, and provides flea adulticide or growth regulation simultaneously <em><strong>or<\/strong><\/em><\/span><\/li>\n<li class=\"li1\"><span class=\"s1\">Dispense a <strong>product indicated for a specific parasite<\/strong>, either tube by tube or box (depending on the predicted growth of the patient); for example, use of a product for tick control in a pediatric patient infested with ticks.<\/span><\/li>\n<\/ul>\n<h3 class=\"p4\"><span class=\"s1\"><b>Fecal Flotation by Centrifugation<\/b><\/span><\/h3>\n<p class=\"p1\"><span class=\"s1\">Ideally, clients should bring a fresh stool sample to each pediatric visit, which allows identification of the protozoal parasites\u2014coccidia and <em>Giardia<\/em>. As mentioned earlier, it is important to convey the zoonotic\/infectious risks that these pathogens possess, which adds value to the test in the pet owner&#8217;s mind.<\/span><\/p>\n<h2 class=\"p3\"><span class=\"s1\"><b>IN SUMMARY<\/b><\/span><\/h2>\n<p class=\"p1\"><span class=\"s1\">This article has focused on preventing infectious and parasitic diseases in pediatric patients. However, those components are only part of a comprehensive wellness plan.
