The most commonly fractured teeth are the canines and carnassials (maxillary fourth premolars and mandibular first molars).<\/span><\/p>\n<p class=\"p1\"><span class=\"s1\">Fractures are further characterized as <em>complicated<\/em> or <em>uncomplicated<\/em>. These types of fractures occur when a piece of the crown is broken, exposing the dentin but not the pulp.<\/span><\/p>\n<p class=\"p1\"><span class=\"s1\">The radiograph (<b>Figure 2<\/b>) reveals that this tooth is nonvital and infected, both of which are evidenced by the periapical rarefaction surrounding all 3 roots (red arrows).<\/span><span class=\"s2\"><sup>2<\/sup><\/span><span class=\"s1\"> Additional radiographic signs of endodontic disease include a wider endodontic space (or on occasion, narrower) and internal or external resorption.<\/span><span class=\"s2\"><sup>2<\/sup><\/span><\/p>\n<div id=\"attachment_3292\" style=\"width: 310px\" class=\"wp-caption alignnone\"><a href=\"https:\/\/todaysveterinarypractice.com\/wp-content\/uploads\/sites\/4\/2013\/03\/Figure-32.jpg\"><img decoding=\"async\" aria-describedby=\"caption-attachment-3292\" class=\"wp-image-3292 size-medium\" src=\"https:\/\/todaysveterinarypractice.com\/wp-content\/uploads\/sites\/4\/2013\/03\/Figure-32-300x214.jpg\" alt=\"Figure 3\" width=\"300\" height=\"214\" srcset=\"https:\/\/navc.sitepreview.app\/todaysveterinarypractice.com\/wp-content\/uploads\/sites\/4\/2013\/03\/Figure-32-300x214.jpg 300w, https:\/\/navc.sitepreview.app\/todaysveterinarypractice.com\/wp-content\/uploads\/sites\/4\/2013\/03\/Figure-32-768x547.jpg 768w, https:\/\/navc.sitepreview.app\/todaysveterinarypractice.com\/wp-content\/uploads\/sites\/4\/2013\/03\/Figure-32.jpg 901w\" sizes=\"(max-width: 300px) 100vw, 300px\" \/><\/a><p id=\"caption-attachment-3292\" class=\"wp-caption-text\">Figure 2<\/p><\/div>\n<p class=\"p1\"><span class=\"s1\"><i>Figures courtesy vetdentalrad.com (Importance of Dental Radiology client educational poster)<\/i><\/span><\/p>\n<div class=\"orange-box\">\n<h2 class=\"p1\"><b>QUESTION &amp; ANSWER<\/b><\/h2>\n<h3 class=\"p2\"><span class=\"s1\"><b>What therapeutic measures should be pursued\u2014keep an eye on the fracture, extract the tooth, or perform a restoration or root canal?<\/b><\/span><\/h3>\n<p class=\"p1\"><span class=\"s1\">Hopefully your answer was not &#8220;keep an eye on the fracture.&#8221; Despite the fact that veterinary patients rarely show clinical signs, uncomplicated crown fractures can be very painful.<\/span><span class=\"s2\"><sup>3<\/sup><\/span><\/p>\n<h3 class=\"p1\"><span class=\"s1\"><b>Role of Dentinal Tubules<\/b><\/span><\/h3>\n<p class=\"p1\"><span class=\"s1\">Dentinal tubules surround the tooth, running from the root canal to the enamel. Each tubule contains an odontoblastic process\u2014basically a nerve supply that is limited to sensory function. There are approximately 50,000 dentinal tubules per mm<sup>2<\/sup> coronal dentin, which is about twice the number found in human teeth.<\/span><span class=\"s2\"><sup>4,5<\/sup><\/span><\/p>\n<p class=\"p1\"><span class=\"s1\">When enamel is lost, the dentin is exposed; therefore, a 1-cm area of enamel loss in a canine tooth exposes 3 to 4 <em>million<\/em> odontoblasts. This exposure leads to quicker dentinal fluid flow out through the dentinal tubules via the capillary effect. The increase in flow deforms the A-delta fibers and C-fibers, which is perceived by the patient as pain.<\/span><span class=\"s2\"><sup>3<\/sup><\/span><\/p>\n<p class=\"p1\"><span class=\"s1\">Factors, such as heat, cold, and desiccation, change the flow rate, cause the nerves to fire, and result in pain (sensitivity).<\/span><span class=\"s2\"><sup>6<\/sup><\/span><span class=\"s1\"> Sensitivity is actually a sign of low-grade pulp inflammation called <em>pulpitis<\/em>.<\/span><\/p>\n<h3 class=\"p1\"><b style=\"line-height: 1.5\">Bacterial Infiltration<\/b><\/h3>\n<p class=\"p1\"><span class=\"s1\">Loss of enamel also allows bacteria to potentially ingress into the root canal system.<\/span><span class=\"s2\"><sup>3,7<\/sup><\/span><span class=\"s1\"> In some cases, these bacteria can result in endodontic infection and subsequent abscessation, which can occasionally manifest clinically as a swelling or draining tract, but is generally subclinical and, therefore, undiagnosed. The only way to definitively diagnose this infection is via dental radiographs.<\/span><\/p>\n<h3 class=\"p1\"><span class=\"s1\"><b>Therapeutic Measures<\/b><\/span><span class=\"s2\"><sup>1,7,8<\/sup><\/span><\/h3>\n<p class=\"p1\"><span class=\"s1\">There are 2 options for nonvital and infected teeth: root canal therapy or extraction.<\/span><\/p>\n<ul class=\"ul1\">\n<li class=\"li1\"><span class=\"s1\">Treatment of infected teeth with root canals has a similar success rate as vital teeth treated with root canals; therefore, root canal therapy should be considered. <\/span><\/li>\n<li class=\"li1\"><span class=\"s1\">If extraction is elected, postoperative dental radiographs should be taken because retained roots are a common complication of these extractions.<\/span><\/li>\n<\/ul>\n<h3 class=\"p1\"><span class=\"s1\"><b>Treatment for this Patient<\/b><\/span><\/h3>\n<p class=\"p1\"><span class=\"s1\">Because the tooth was nonvital and infected, I elected to perform a root canal, based on the success of this therapy in my experience. If root resorption had been seen on the radiograph, extraction would have been the selected therapy.<\/span><\/p>\n<h3 class=\"p1\"><span class=\"s1\"><b>Bonded Sealant Therapy<\/b><\/span><\/h3>\n<p class=\"p1\"><span class=\"s1\">If no radiographic evidence of disease had been present in this case, the tooth would have been treated with a bonded sealant.<\/span><span class=\"s2\"><sup>9<\/sup><\/span><span class=\"s1\"> This therapy resolves sensitivity, blocks off the pathway for infection, improves aesthetics, and smooths the tooth to decrease plaque
