This article discusses the rationale for performing ear cytology and covers the step-by-step process of collecting, staining, and evaluating samples.<\/p>\n<h2 class=\"p1\">When to Perform Ear Cytology<\/h2>\n<p class=\"p2\"><span class=\"s1\">Evaluation of clinical signs related to otitis externa (<\/span><span class=\"s2\"><b>BOX 1<\/b><\/span><span class=\"s1\">) is one of the most common reasons animals present to the clinic.<sup>2<\/sup> Ear cytology is a simple, quick, routine procedure that can be used to rule out or identify microbial otic infection in any animal that presents with these signs.<sup>3<\/sup> With antimicrobial resistance on the rise, selective treatment options are critical in combating infectious organisms. A skilled veterinary nurse can enhance the veterinarian\u2019s diagnosis by performing ear cytology and correctly identifying microorganisms by group. This, in turn, helps guide the veterinarian\u2019s choice of the most effective therapy or further tests, such as culture and sensitivity.<sup>3,4<\/sup> Ear cytology is also an invaluable tool in monitoring therapy, with rechecks performed weekly or biweekly as treatment progresses until there is no evidence of infection on cytology samples.<sup>2,3<\/sup><\/span><\/p>\n<p class=\"p1\"><div class=\"su-box su-box-style-default\" id=\"\" style=\"border-color:#606060;border-radius:0px;\"><div class=\"su-box-title\" style=\"background-color:#939393;color:#FFFFFF;border-top-left-radius:0px;border-top-right-radius:0px\">BOX 1 Common Signs of Otitis Externa<\/div><div class=\"su-box-content su-u-clearfix su-u-trim\" style=\"border-bottom-left-radius:0px;border-bottom-right-radius:0px\">\n<ul>\n<li class=\"p1\">Head shaking<\/li>\n<li class=\"p1\">Ear scratching<\/li>\n<li class=\"p1\">Erythema of the ear<\/li>\n<li class=\"p1\">Pain when ears are massaged or manipulated<\/li>\n<li class=\"p1\">Foul odor<\/li>\n<li class=\"p1\">Discharge within the ear canal<\/li>\n<\/ul>\n<p class=\"p1\"><\/div><\/div>\n<h2 class=\"p1\">Causes of Otitis Externa<\/h2>\n<p class=\"p2\"><span class=\"s1\">A classification system known as <i>PSPP<\/i> is used to <\/span>describe causes of otitis externa. PSPP stands for <i>primary<\/i><span class=\"s1\"><i>, secondary, predisposing, and perpetuating factors<\/i>.<sup>2<\/sup> Ear cytology plays an important role in identifying primary, secondary, and perpetuating factors.<\/span><\/p>\n<p class=\"p2\"><span class=\"s1\"><b>Primary factors<\/b> are those that cause infections in healthy ears. Specific primary causes detectable by ear sampling include parasites such as <i>Otodectes cynotis <\/i>(ear mite of dogs and cats), <i>Psoroptes cuniculi<\/i> (ear mite of rabbits and goats), and <i>Otobius megnini<\/i> (spinose ear tick of dogs). Less common but documented parasitic primary causes of otitis externa include <i>Demodex <\/i>species,<i> Notoedres cati, Sarcoptes scabiei, Cheyletiella <\/i>species,<i> Eutrombicula alfreddugesi, <\/i>and<i> Neotrombicula autumnalis<\/i>.<sup>2-5<\/sup> <\/span><\/p>\n<p class=\"p2\"><span class=\"s1\"><b>Secondary factors<\/b> include opportunistic bacteria and yeast overgrowth, most commonly by <i>Malassezia pachydermatis. <\/i>These organisms<i> <\/i>are often detected by ear sampling and culture and sensitivity.<sup>2-5<\/sup> <\/span><\/p>\n<p class=\"p2\"><span class=\"s1\"><b>Predisposing factors<\/b> for otitis externa include ear conformation and anatomy conducive to infection, such as pendulous pinnae, hypertrichosis, and stenotic ear canals. Environmental predisposing factors include humid, moist environments from frequent swimming, bathing, or excessive ear cleaning. Other predisposing factors include systemic conditions such as immunosuppression or hypoadrenocorticism and obstructions (e.g., polyps, neoplastic growths).<sup>2,3,5<\/sup> <\/span><\/p>\n<p class=\"p2\"><span class=\"s1\"><b>Perpetuating factors<\/b> continue the disease and include opportunistic bacteria, yeast overgrowth, otitis media, cellular hyperplasia, ear canal edema, calcification, and excessive fluid or debris within the middle ear and ear canal.<sup>2-5<\/sup> <\/span><\/p>\n<h2 class=\"p1\">Sample Collection and Preparation<\/h2>\n<p class=\"p2\"><span class=\"s1\">Specimens from the external ear canal are collected during physical examination and otoscopic examination of the ear. Ideally, samples are collected before the addition of any aural cleansers or medications.<sup>2-5<\/sup> In some cases of otitis media with intact tympanic membranes, a skilled veterinarian may perform a myringotomy to collect a culture for bacterial analysis.<sup>5,6<\/sup> Supplies needed for sample collection and analysis are listed in <\/span><span class=\"s2\"><b>BOX 2<\/b><\/span><span class=\"s1\">.<\/span><\/p>\n<p class=\"p2\"><span class=\"s1\"><div class=\"su-box su-box-style-default\" id=\"\" style=\"border-color:#606060;border-radius:0px;\"><div class=\"su-box-title\" style=\"background-color:#939393;color:#FFFFFF;border-top-left-radius:0px;border-top-right-radius:0px\">BOX 2 Supplies Needed for Ear Cytology<\/div><div class=\"su-box-content su-u-clearfix su-u-trim\" style=\"border-bottom-left-radius:0px;border-bottom-right-radius:0px\"><\/span><\/p>\n<ul>\n<li class=\"p2\">Otoscope<\/li>\n<li class=\"p2\">Cotton-tipped swabs<\/li>\n<li class=\"p2\">Microscope slides (frosted edge preferred for clear and quick labeling)<\/li>\n<li class=\"p2\">Mineral oil for mite identification<\/li>\n<li class=\"p2\">Romanowsky-type cytology stains (a dedicated cytology stain set is preferred to avoid contamination of stain solutions used for blood and fine-needle aspirate slides with yeast and bacteria)<sup>3<\/sup><\/li>\n<li class=\"p2\">Microscope with 100\u00d7 objective<\/li>\n<\/ul>\n<\/div><\/div>\n<h3 class=\"p3\">Step 1<\/h3>\n<p class=\"p2\"><span class=\"s1\">An otoscope is used to examine the ear to note physical characteristics such as exudate accumulation, presence of mites or ticks, and the condition of the tympanic membrane (intact or ruptured). <\/span><\/p>\n<h3 class=\"p3\">Step 2<\/h3>\n<p class=\"p2\"><span class=\"s1\">Each ear is sampled with a separate swab to determine if infections are bilateral or unilateral.<sup>3,4<\/sup> A dry swab is inserted into the ear deep enough to reach the junction between the horizontal and vertical ear canals and rotated several times (<\/span><span class=\"s2\"><b>FIGURE 1<\/b><\/span><span class=\"s1\">).<sup>3<\/sup> Samples are more diagnostic if taken from the horizontal ear canal; however, take care not to pull the ear. In general, the deeper an infection goes, the more painful it is.<sup>6<\/sup> If the tympanic membrane has ruptured, a sterile sample from the middle ear may be needed to compare against samples from the horizontal ear canal on culture.<sup>2<\/sup> <\/span><\/p>\n<h3 class=\"p3\">Step 3<\/h3>\n<p class=\"p2\"><span class=\"s1\">The swabs are gently rolled on a clean slide, with complete revolutions of the swab tip (<\/span><span class=\"s2\"><b>FIGURE 2<\/b><\/span><span class=\"s1\">).<sup>4<\/sup> Material must not be painted on the slide, but gently transferred during the rolling process. Collected material is gently rolled onto a slide, using complete revolutions of the swab to transfer material. \u201cPainting\u201d a slide causes cell rupture, and areas will be too thick to evaluate.<\/p><\/div>\n<p class=\"p2\"><span class=\"s1\">The author prefers to start with the right ear and roll the swab in a single line across the top of the slide, beginning at the side next to the frosted edge. The left ear is sampled in the same manner with a clean swab and rolled in a single line across the same slide below the first line. Placing the exudate in one line on the slide allows for more thorough evaluation by limiting the stage movement necessary to evaluate the entire sample thoroughly and ensures a thin area of evaluation for easier cellular and microorganism identification. Lines are labeled using \u201cL\u201d for left and \u201cR\u201d for right. Patient name, owner information, and date are also added to the label. <\/span><\/p>\n<h3 class=\"p3\">Step 4<\/h3>\n<p class=\"p2\"><span class=\"s1\">Samples are allowed to air dry before staining with quick stains according to the stain manufacturer\u2019s instructions. Fixation of cells to the slide by heat is unnecessary.<sup>3,4,7<\/sup> Using methanol as a fixative is sufficient to fix any microorganisms to the slide before staining. Unnecessary heat causes dehydration of cells and may impede their identification and microorganisms within them. <\/span><\/p>\n<h3 class=\"p3\">Step 5<\/h3>\n<p class=\"p2\"><span class=\"s1\">Once slides are stained and air dried, they are carefully scanned under the 40\u00d7 objective for cells and larger structures. Bacteria are further evaluated using the 100\u00d7 objective.<sup>3<\/sup> <\/span><\/p>\n<p class=\"p2\"><span class=\"s1\">If mites are suspected, a second swab with mineral oil applied is used to collect exudate. This material is mixed into a drop of mineral oil on a separate slide. Mite evaluation slides are not stained. Mineral oil added to the swab helps collect and trap mites for evaluation. 100\u00d7 magnification, unstained.<\/p><\/div>\n<p class=\"p2\"><span class=\"s1\">A deep skin scraping of the pinna may be necessary to recover other suspected mites such as <i>Demodex <\/i>species, <i>S. scabiei<\/i>, or <i>N. cati<\/i>. The pinna is pinched, scraped with a scalpel blade coated with mineral oil, released, pinched again, and scraped until capillary blood oozes from the site. This material is mixed with a drop of mineral oil on a slide, cover-slipped, and examined under the 10\u00d7 objective for mites and\/or mite ova.<sup>7<\/sup> <\/span><\/p>\n<h2 class=\"p1\">Reporting Cytology Findings<\/h2>\n<p class=\"p2\"><span class=\"s1\">Bacteria in ear cytology samples are often described based on morphology (rods versus cocci) and arrangement (e.g., clusters, pairs, chains, tetrads).<sup>3<\/sup> Bacterial numbers may be reported as average number per 40\u00d7 objective; as occasional, mild, moderate, or marked; or by using a scale of 1+, 2+, 3+, etc. to denote increasing numbers. These classifications may depend on patient species. For example, in dogs, an average of &gt;25 bacteria per 40\u00d7 field may be reported as \u201cmarked;\u201d in cats, a \u201cmarked\u201d number may be &gt;15 bacteria per 40\u00d7 field.<sup>3<\/sup><\/span><\/p>\n<p class=\"p5\"><span class=\"s1\">An alternative reporting scheme grades ear cytology samples based on the presence of bacteria, yeast, and inflammatory cells by semi-quantitative methods:<sup>8<\/sup><\/span><\/p>\n<p class=\"p6\"><span class=\"s1\"><strong>0 \u2014<\/strong> No bacteria, yeast, or inflammatory cells are observed.<\/span><\/p>\n<p class=\"p6\"><span class=\"s1\"><strong>1 \u2014<\/strong> Occasional bacteria, yeast, or inflammatory cells are present but only found through careful screening.<\/span><\/p>\n<p class=\"p6\"><span class=\"s1\"><strong>2 \u2014<\/strong> Low numbers of bacteria, yeast, or inflammatory cells are present and easily found without difficulty.<\/span><\/p>\n<p class=\"p6\"><span class=\"s1\"><strong>3 \u2014<\/strong> Larger numbers of bacteria, yeast, or inflammatory cells are present and easily found without difficulty.<\/span><\/p>\n<p class=\"p7\"><span class=\"s1\"><strong>4 \u2014<\/strong> Massive amounts of bacteria, yeast, or inflammatory cells are present and easily found without difficulty.<\/span><\/p>\n<p class=\"p2\"><span class=\"s1\">Specific reporting schemes should be documented in standard operating procedures developed by each clinic. Proteinaceous material and rod bacteria are also present. 1000\u00d7 magnification, Romanowsky-type stains.<\/p><\/div>\n<p class=\"p2\"><span class=\"s1\">Bacterial rods are most likely gram negative, and their presence in canine or feline ear cytology samples is considered abnormal, signaling the need for culture and sensitivity testing (<\/span><span class=\"s2\"><b>FIGURE 4<\/b><\/span><span class=\"s1\">).<sup>3,5,6,9<\/sup> <i>Pseudomonas aeruginosa<\/i> is the most common rod-shaped bacteria found during ear cytology, with <i>Proteus mirabilis<\/i> and coliform bacteria such as <i>Escherichia coli <\/i>occasionally cultured<i>.<\/i><sup>2,4-6,9<\/sup> <i>Pseudomonas <\/i>species produce endotoxins and are surrounded by a glycolipid matrix that protects against the animal\u2019s immune system and topical antimicrobials.<sup>6 <\/sup><\/span><\/p>\n<h3 class=\"p3\">Yeast and Fungi<\/h3>\n<p class=\"p2\"><span class=\"s1\"><i>Malassezia pachydermatis<\/i> appears as a peanut-, snowman-, or shoeprint-shaped, budding yeast that usually stains intensely purple with Romanowsky stains (<span class=\"s2\"><b>FIGURE<\/b><\/span><\/span><span class=\"s2\"><b>\u00a04<\/b><\/span><span class=\"s1\">).<sup>3,9<\/sup> <i>Malassezia<\/i> may be reported similarly to bacteria, with average numbers per 40\u00d7 objective recorded. Fewer than 2 <i>Malassezia<\/i> organisms per 40\u00d7 field is considered typical.<sup>3<\/sup> To report a marked number of <i>Malassezia<\/i> present, there should be &gt;5 organisms per 40\u00d7 objective field in dogs and &gt;12 per 40\u00d7 objective field in cats.<sup>3<\/sup> <\/span><\/p>\n<p class=\"p2\"><span class=\"s1\">Less common fungi found in ear cytology samples include <i>Candida<\/i> and <i>Microsporum <\/i>species.<sup>3<\/sup> Fungal culture is used to further identify any yeast or hyphae that cannot readily be identified microscopically.<sup>3<\/sup><\/span><\/p>\n<h3 class=\"p3\">Cells<\/h3>\n<p class=\"p2\"><span class=\"s1\">Inflammatory cells include neutrophils and macrophages, neither of which is typically present in healthy ears.<sup>3<\/sup> Neutrophils are described as nondegenerate or degenerate. Nondegenerate neutrophils have the typical morphology of a mature segmented neutrophil found in peripheral blood.<sup>10<\/sup> Degenerate neutrophils indicate that neutrophils have been damaged by their environment, most often by endotoxin-producing bacteria.<sup>10<\/sup> Degenerate neutrophils resemble large band cells due to an inability to control water homeostasis.<sup>10<\/sup> These cells swell, especially nuclei, and their cytoplasm may appear foamy or vacuolated.<sup>10<\/sup> Most neutrophils found in ear cytology slides are degenerate and associated with <i>Pseudomonas<\/i> infections.<sup>6,9<\/sup> If macrophages are present, it is important to examine them closely for bacteriophagy, which may indicate pathogenicity of any bacteria present.<sup>3<\/sup> Inflammatory cells are often accompanied by the presence of proteinaceous material, which appears as a pale pink to purple in the background of the slide (<span class=\"s2\"><b>FIGURES<\/b><\/span><\/span><span class=\"s2\"><b> 4 AND 5<\/b><\/span><span class=\"s1\">).<sup>5 <\/sup><\/span><\/p>\n<p class=\"p2\"><span class=\"s1\">Neoplastic cells can develop within the ear canal as they do in skin samples.<sup>3<\/sup> Additional samples may be taken by fine-needle aspiration, laser biopsy, or skin scraping to identify specific cells. Acantholytic epithelial cells appear rounded with a densely stained cytoplasm and are often associated with chronic inflammatory skin diseases such as pemphigus.<sup>3,10<\/sup><\/span><\/p>\n<h3 class=\"p3\">Substances<\/h3>\n<p class=\"p2\"><span class=\"s1\">Melanin may be confused with bacteria and is a common finding on ear cytology slides. It appears <\/span>golden-yellow to brown on Romanowsky stains, whereas<span class=\"s1\"> bacteria appear dark purple. Melanin granules are randomly arranged within cells, as opposed to bacteria, <\/span>which are more organized. Melanin granules also appear<span class=\"s1\"> highly refractive as the microscope focus is adjusted.<sup>9<\/sup><\/span><\/p>\n<p class=\"p2\"><span class=\"s1\">Cerumen is a fat-laden exudate secreted by ceruminous glands of the external ear canal and is a common finding in ear cytology samples.<sup>3<\/sup> Because of its high fatty content, it resists staining and appears as a brownish material in stained ear cytology slides.<sup>3<\/sup><\/span><\/p>\n<h2 class=\"p1\">Conclusion<\/h2>\n<p class=\"p2\"><span class=\"s1\">Ear sampling and cytologic evaluation is a rapid, inexpensive test that a skilled veterinary nurse can perform in house to identify parasites, cocci and rod bacteria, inflammatory cells, and yeast organisms, which are common in otitis externa. This evaluation process is invaluable in guiding the veterinarian in his or her initial choice of treatment and ongoing evaluation of treatment effectiveness. As the point person for client education, the veterinary nurse must relay the importance of initial and follow-up ear cytology testing to pet owners to facilitate their acceptance and compliance. It is also another way veterinary nurses can use their skills to add value to the role they play in patient health and practice productivity.
